The most common bosutinib side effects in patients with CML are diarrhea, respiratory tract infection, nausea, rash, abdominal pain, vomiting, fatigue, pyrexia, headache, and hepatic dysfunction.

  • Therapy with bosutinib may cause elevations in serum transaminases (alanine aminotransferase, aspartate aminotransferase-AST). Patients receiving this medication should have liver function tests before treatment initiation and monthly for the first three months of treatment, and then as clinically required.

Manage all patients with transaminase elevations by withholding Bosutris tablet temporarily, and/or discontinuation of treatment. Elevations of transaminases, mainly in the setting of concomitant rises in bilirubin, may be an advance indication of drug-induced liver injury and these patients need to be managed appropriately.

Bosutinib treatment may cause diarrhea and vomiting; for that reason, patients with a recent or ongoing clinically significant gastrointestinal disorder should use this drug with caution and only after a precise benefit-risk evaluation. Patients with vomiting and diarrhea should be managed through standard-of-care treatment, including an antiemetic or antidiarrhoeal medicinal product or fluid replacement.

Bosutinib therapy may cause myelosuppression, defined as anemia, thrombocytopenia, and neutropenia. Perform complete blood counts weekly for the initial month and then monthly thereafter, or as clinically required. Manage myelosuppression by withholding bosutinib 100 mg, 400 mg, 500 mg temporarily, dose reduction, or interrupting bosutinib treatment.

Bosutinib may cause fluid retention including pleural effusion, pericardial effusion, pulmonary edema, or peripheral edema. Monitor and manage patients using standard-of-care treatment. In addition, manage fluid retention by withholding bosutinib temporarily, dose reduction, and/or interrupting bosutinib.

Elevation in serum lipase may occur. Caution is required in those with a previous history of pancreatitis. In case the lipase elevations are accompanied by abdominal symptoms, interrupt bosutinib and consider appropriate diagnostic measures in order to exclude pancreatitis.
