Flucytosine is an antifungal agent, available as 250 mg and 500 mg tablets for oral administration. The Flucytosine (5 flucytosine) tablets are widely used only to treat serious infections caused by susceptible strains of Candida and/or Cryptococcus.

Flucytosine therapy should be used in combination with medicine amphotericin B to treat systemic candidiasis and cryptococcosis due to emergence of resistance to flucytosine.

The standard dosage of flucytosine tablets is 50 to 150 mg/kg/day given in splitted doses at 6 hour intervals. Vomiting or nausea may be reduced or avoided if the pills are given a few at a time over a 15-minute time-span. In case the BUN or the serum creatinine is elevated, or if there are other signs/symptoms of renal impairment, the initial dose needs to be at the lower level. 

The common flucytosine side effects are nausea, low potassium levels in the blood, vomiting, diarrhea, skin sensitivity, stomach ache, inflammatory bowel disease, headache, drowsiness, fever and vertigo. 

  • Flucytosine tablets must be used with extreme caution in all those with impaired renal function. Since this medicinal product is excreted primarily through kidneys, renal impairment may lead to accumulation of the medicine.
  • Flucytosine serum concentrations need to be evaluated to specify the adequacy of renal excretion in such patients. Dosage adjustments are required in those with renal insufficiency in order to prevent the progressive accumulation of active medicine.
  • Cytoflu or cytorx (Flucytosine) must be used with extreme caution in those with bone marrow depression. Respective individuals may be more prone to depression of bone marrow function if they have a hematologic disease.
  • Bone marrow toxicity (BMT) can be irreversible and may lead to death in those who are immunosuppressed. Frequent evaluation of hepatic function and of the hematopoietic system is required throughout the therapy.
  • Before Flucytosine therapy is instituted, determine the electrolytes (due to hypokalemia) and the hematologic and renal status of the patient. Close assessment of the patient throughout the therapy is essential. 
  • Since renal impairment can cause progressive accumulation of the drug, monitor kidney function and blood concentrations during therapy. Determine leukocyte and thrombocyte count and liver function at frequent intervals throughout the treatment as required.
  • There are no adequate and well-controlled findings/studies in pregnant women. Flucytosine 500 mg treatment should be considered during pregnancy only in case the potential advantages justifies the potential risk to unborn baby or fetus.
  • It is not specified whether Flucytosine is excreted in human milk. Because many medicines are excreted in human milk and because of the risk for serious side effects in nursing infants from Flucytosine, do not use this medication unless critically required.
