Heparin 5000 IU/25,000 IU/5mL is an anticoagulant used for:

  • Prophylaxis, venous thromboembolism and pulmonary embolism

The heparin injection should be given by intermittent intravenous injection, intravenous infusion, or deep subcutaneous (intrafat, i.e., above the iliac crest or abdominal fat layer) injection. Avoid the intramuscular route of administration due to the frequent occurrence of hematoma at the site of injection. 

Although dosage must be adjusted for the individual patients as per the results of suitable laboratory tests, the following dosage schedules may be used as guidelines:

Paediatric Population: The suggested dose of heparin is 50 IU per kg body weight initially given by intravenous infusion, followed by 100 IU per kg body weight every 4 hours (as per clotting time) or continuous infusion of 20,000 IU/m2/24 hours. The common side effects with Heparin injection are hemorrhage, HIT and HITT, thrombocytopenia, hypersensitivity reactions, and elevations of aminotransferase levels.

  • Do not use heparin 5000 or any other dosage strength in the existence of major bleeding, except when the benefits of heparin treatment outweigh the potential risks. Hemorrhage, including fatal events, may occur in patients receiving Heparin Sodium. Hemorrhage may occur at virtually any site in all patients receiving heparin. 
  • Heparin comes in various strengths. Fatal hemorrhages have occurred due to medicine errors. Precisely examine all heparin products to confirm the correct container choice before the administration of the medication. 
  • HIT may occur in patients treated with heparin 25000. Monitor thrombocytopenia of any degree precisely. In case the platelet count falls below 100,000/mm3 or if recurrent thrombosis occurs, promptly discontinue therapy, assess for HIT and HITT, and, if required, administer an alternative anticoagulant.
  • When using a full dose regimen of heparin, adjust the dose of heparin depending on frequent blood coagulation tests. If the coagulation test is unduly prolonged or if hemorrhage occurs, heparin uses should be interrupted promptly. Periodic platelet counts, hematocrits are required throughout heparin therapy. 
  • Patients with documented hypersensitivity to heparin should be prescribed the medicine only in clearly life-threatening situations. Because heparin sodium is derived from animal tissue, monitor for signs/symptoms of hypersensitivity when it is used in those with a known history of allergy. 
  • Patients should avoid the consumption of alcohol with Heparin injection as it may be responsible for causing severe bleeding in the stomach and hazardous adverse reactions to the liver.
