Piclib capsules for oral administration contain 125 mg of palbociclib, a kinase inhibitor. Palbociclib 125mg and aromatase inhibitor are taken by mouth (orally) in a 4-week cycle as a once-daily dose. For the initial 3 weeks (21 days), take one palbociclib and one aromatase inhibitor pill orally once daily. Then for the last week (7 days), take only one pill once daily- an aromatase inhibitor. At the end of 4 weeks (28 days), start the cycle again.

Even though the patient stops taking Piclib for the last week of each cycle, an aromatase inhibitor should be taken every day throughout the therapy.

When given with Piclib (palbociclib dosage), the proposed dose of fulvestrant is 500 mg administered on Days 1, 15, 29, and once monthly thereafter. 

Take palbociclib capsules with food. Swallow the pills as a whole (do not chew, crush, or open them prior to swallowing). Pills must not be ingested in case they are broken, cracked, or otherwise not intact.

If any patient vomits or misses a dose, an additional dose should not be taken. The very next prescribed dose should be taken at the usual time.

The most common PICLIB- palbociclib side effects are neutropenia, infections, leukopenia, decreased appetite, fatigue, stomatitis, anemia, alopecia, diarrhea, thrombocytopenia, rash, vomiting, asthenia, nausea, and pyrexia.  

  • Pre/perimenopausal women who received treatment with the combination of Palbociclib plus fulvestrant therapy should also be treated with luteinizing hormone-releasing hormone (LHRH) agonists as per the current clinical practice standards.
  • For males who receive treatment with a combination of Ibrance 125 mg plus aromatase inhibitor therapy, consider treatment with a luteinizing hormone-releasing hormone (LHRH) agonist as per the current clinical practice standards.
  • Neutropenia may occur in patients with palbociclib treatment. Monitor complete blood counts prior to initiating this treatment and at the initiation of each cycle, as well as on Day 15 of the first two cycles. Dose interruption/reduction/delay in starting treatment cycles is required for those who develop Grade 3/4 neutropenia.
  • Piclib/ibrance palbociclib pills can cause fetal harm when administered to a pregnant woman. The respective women should use appropriate contraception during treatment with targeted therapy and for at least 3 weeks (21 days) after the final dose.
  • There is no deets regarding the availability of palbociclib in milk, its effects on the breastfed infant, or milk production. Due to the potential for severe side reactions in breastfed infants from Piclib, lactating women should not breastfeed during treatment and for three weeks after the final dose.
