Dosage and Side Effects

The recommended dosage of Abciximab injection in adult patients is a 0.25 mg/kg intravenous (IV) bolus administered 10 to 60 minutes prior to starting percutaneous coronary intervention (PCI), followed by a continuous intravenous (IV) infusion of 0.125 µg/kg/min (to a maximum of 10 µg/min) for 12 hours.

Patients with unstable angina who do not respond to conventional treatment and who are scheduled to undergo percutaneous coronary intervention (PCI) within 24 hours may be treated with a 0.25 mg/kg intravenous (IV) bolus followed by an 18-24-hour intravenous (IV) infusion of 10 µg/min, concluding one hour after the percutaneous coronary intervention (PCI).

The most common side effects of Abciximab injection are bleeding, back pain, nausea, hypotension, and chest pain.

Warning and Precautions

  • Abciximab may increase the probability of bleeding, mainly in the existence of anticoagulation, e.g., from heparin, thrombolytics, or anticoagulants. The probability of major bleeding because of this drug may be increased in those receiving thrombolytics and should be weighed against the anticipated benefits.

  • Treatment with Abciximab 5mL requires close attention to all potential bleeding sites (including cutdown sites, catheter insertion sites, arterial and venous puncture sites, needle puncture sites, and genitourinary, gastrointestinal, and retroperitoneal sites).

  • Minimize arterial and venous punctures, IM injections, and use of urinary catheters, nasogastric tubes, nasotracheal intubation, and automatic blood pressure cuffs. When obtaining IV access, non-compressible sites need to be avoided. Document and monitor vascular puncture sites. Provide gentle care when removing dressings.

  • Monitor platelet counts prior to treatment, 2 to 4 hours following the bolus dosage of Abciximab injection, and at 24 hours or before discharge, whichever is first. In case a patient experiences an acute platelet decrease, additional platelet counts need to be determined.

  • In the event of critical uncontrolled bleeding or the need for emergency surgery, treatment with Abciximab needs to be discontinued. In case platelet function does not return to normal, it may be restored, at least in part, with platelet transfusions.
