Hepcvir L Tablet, a generic version of Harvoni, is a fixed-dose combination (FDC) of ledipasvir, a hepatitis C virus (HCV) NS5A inhibitor, and sofosbuvir, an HCV nucleotide analog NS5B polymerase inhibitor, and is used for the treatment of chronic hepatitis C virus (HCV) in adult and pediatric patients aged three years and older:

  • Genotype 1, 4, 5, and/or 6 infection either without cirrhosis or with compensated cirrhosis.
  • Genotype 1 infection with decompensated cirrhosis, together with ribavirin.
  • Genotype 1 and/or 4 infection who are liver transplant recipients either without cirrhosis or with compensated cirrhosis, together with ribavirin.

Dosage and Side Effects

The recommended dosage of Hepcvir L in adult patients with genotype 1, 4, 5, or 6 HCV is one tablet (90 mg ledipasvir and 400 mg sofosbuvir) taken orally once daily with or without food. The daily dosage of ribavirin is weight-based (1000 mg for those <75 kg and 1200 mg for those ≥75 kg) administered orally in 2 divided doses with food. In those with decompensated cirrhosis, the starting dosage of ribavirin is 600 mg and may be titrated up to 1000 mg for those <75 kg and 1200 mg for those ≥75 kg in 2 divided doses with food.

The recommended dosage of Hepcvir L in pediatric patients aged 3 years and older with genotype 1, 4, 5, or 6 HCV using Hepcvir L tablets is based on weight. Pediatric patients weighing at least 35 kg should take one 90 mg/400 mg tablet once daily, patients weighing 17 kg to not more than 35 mg should take one 45 mg/200 mg tablet once daily, patients weighing less than 17 kg should take 33.75 mg/150 mg daily orally with or without food.

The most common side effects of Ledipasvir 90 mg; and Sofosbuvir 400 mg tablets are headache fatigue, and asthenia.

Warning and Precautions

  • Test all patients for evidence of ongoing or prior HBV infection by measuring hepatitis B surface antigen (HBsAg) and hepatitis B core antibody (anti-HBc) prior to initiating HCV treatment with Hepcvir L tablets.
  • Cases of symptomatic bradycardia, as well as serious cardiac arrest and cases that may require pacemaker intervention, may occur when amiodarone is coadministered with Hepcvir L tablets. Coadministration of amiodarone with Hepcvir L is not recommended. 
  • The concomitant use of Ledipasvir; Sofosbuvir and P-gp inducers (such as rifampin and St. John’s wort) may significantly scale down the plasma concentrations of ledipasvir and sofosbuvir and may lead to a decreased therapeutic effect of Hepcvir L tablets. Therefore, the use of this drug with P-gp inducers (such as rifampin or St. John’s wort) is not recommended.
  • Patients taking treatment with Hepcvir L who develop signs or symptoms of bradycardia should seek prompt medical evaluation. Symptoms may include shortness of breath, near-fainting or fainting, chest pains, dizziness or lightheadedness, malaise, weakness, tiredness, confusion, or memory problems.
