Description

Pomalidomide is a thalidomide analogue with immunomodulatory, antineoplastic, and antiangiogenic properties. This therapeutic drug is used for the treatment of adult patients with:

  • multiple myeloma (MM), taken along with the medicine dexamethasone, in patients who have previously received at least two medications to treat MM, including a proteasome inhibitor and medicine lenalidomide, and whose condition has become worse during therapy or within 60 days of finishing the last therapy.
  • AIDS-related Kaposi sarcoma (KS) following failure of highly active antiretroviral therapy (HAART) or in patients with KS who are HIV-negative. (This indication of Pomalidomide is approved under accelerated approval on behalf of the overall response rate).

Dosage and Side Effects

The recommended dosage of Pomalidomide in patients with Multiple Myeloma (MM) is 4 mg once daily, and in patients with Kaposi Sarcoma (KS) is 5 mg once daily, administered orally with or without food on Days 1 through 21 of each 28-day cycle until disease progression. For multiple myeloma, pomalidomide is used along with medicine dexamethasone. HAART should be continued as HIV therapy in patients with AIDS-related Kaposi sarcoma (KS).

The most common side effects of Pomalidomide capsules in patients with Multiple Myeloma (MM) and AIDS-related Kaposi sarcoma (KS) are as:

MM: fatigue and asthenia, anemia, constipation, neutropenia, nausea, diarrhea, upper respiratory tract infections, dyspnea, back pain, and pyrexia.

KS: abnormal kidney function tests, tiredness, diarrhea, decreased phosphate and calcium in the blood, rash, constipation, nausea, increased blood sugar, and decreased albumin in the blood.

Warning and Precautions

  • Females of reproductive age/potential must have negative pregnancy testing and use effective or appropriate contraception methods prior to initiating treatment.
  • Patients must avoid donating blood during treatment with Pomalidomide and for 4 weeks after discontinuation of the treatment because the blood might be given to a pregnant female patient whose fetus must not be exposed to medicine.
  • Venous thromboembolic events and arterial thromboembolic events may occur in those treated with Pomalidomide 4 mg/3 mg/2 mg/1 mg capsules. Patients with known risk factors, including before thrombosis, may be at higher risk, and actions should be taken to minimize all modifiable factors (e.g., hyperlipidemia, smoking, hypertension).
  • Monitor patients for hematologic toxicities, mainly neutropenia. Monitor complete blood counts (CBC) weekly for the initial eight weeks and monthly thereafter. Patients may require either dose interruption or dose modification.
  • Hepatic failure, including fatal cases, may occur in patients treated with Pomalidomide. Elevated levels of alanine aminotransferase and bilirubin also may occur in patients treated with this drug. Assess liver function tests monthly. Stop therapy upon elevation of liver enzymes and evaluate.
  • Tumor lysis syndrome (TLS) may develop in patients treated with this medicinal product. Patients at risk for TLS are all those with high tumor burden before treatment. These patients need to be monitored precisely and relevant precautions taken.
