Cetuximab is an epidermal growth factor receptor (EGFR) antagonist used for the treatment of:

Head and Neck Cancer:

  • Locally or regionally advanced squamous cell carcinoma of the head and neck along with radiation therapy.
  • Recurrent locoregional disease or metastatic squamous cell carcinoma of the head and neck along with platinum based therapy with fluorouracil.
  • Recurrent or metastatic squamous cell carcinoma of the head and neck progresses after platinum-based therapy.

K-Ras wild-type, EGFR-expressing, Metastatic Colorectal Cancer:

  • In conjunction with FOLFIRI for first-line treatment,
  • In conjunction with irinotecan in those who are refractory to irinotecan-based chemotherapy,
  • As a monotherapy in those who have failed oxaliplatin- and irinotecan-based chemotherapy or who are intolerant to irinotecan.

Dosage and Side Effects

The recommended initial dose is 400 mg/m2 administered as a 120-minute intravenous (IV) infusion followed by 250 mg/m2 weekly infused over 60 minutes. Start Cetuximab one week before initiation of radiation therapy. Complete Cetuximab administration one hour before irinotecan, platinum-based therapy along with fluorouracil or FOLFIRI.

The most common side effects of Cetuximab are cutaneous adverse reactions (including rash, pruritus, and nail changes), infection headache, and diarrhea.

Warning and Precautions

  • Serious and potentially fatal infusion reactions may occur with Cetuximab 100 mg/50 mL. Patients should be monitored for at least one hour post-infusion in a facility equipped to manage anaphylaxis. If infusion reactions occur, monitoring should continue for more than one hour until resolution.
  • Exercise caution when using Cetuximab alongside radiation therapy or platinum-based therapy with fluorouracil in patients with squamous cell carcinoma of the head and neck (SCCHN) and a history of coronary artery disease, congestive heart failure, or arrhythmias, as it may lead to cardiopulmonary arrest. Regularly monitor serum electrolyte levels, including magnesium, potassium, and calcium, during and after Cetuximab treatment.
  • Be vigilant for signs of interstitial lung disease (ILD) during Cetuximab treatment. If patients exhibit acute or worsening pulmonary symptoms, consider interrupting or discontinuing treatment accordingly. Confirmed ILD warrants permanent cessation of Cetuximab therapy.
  • Patients undergoing treatment with Cetuximab may experience dermatologic side effects such as acneiform rash, dry skin, nail inflammation, and increased hair growth. Regularly monitor patients for these issues and advise them to minimize sun exposure while on Cetuximab therapy.
