Sofosbuvir 400 MG Tablet, a hepatitis C virus (HCV) nucleotide analog NS5B polymerase inhibitor, is supplied under the generic brand name SoviHep. This therapeutic medicine is widely used for the treatment of adults with genotype 1/2/3/4 chronic hepatitis C virus infection without cirrhosis or with compensated cirrhosis as a component of a combination antiviral treatment regimen, and pediatric patients aged 3 years and older with genotype 2/3 chronic hepatitis C virus infection without cirrhosis or with compensated cirrhosis in conjunction with ribavirin.

Dosage and Side Effects

The recommended dosage is one 400 mg tablet of Sofosbuvir, taken orally, once daily with or without food. It is taken along with ribavirin or along with pegylated interferon and ribavirin for treating patients with hepatitis C virus (HCV).

The recommended dose in pediatric patients aged 3 years and older with genotype 2/3 hepatitis C virus depends on body weight. Patients weighing at least 35 kg can take one 400 mg tablet once daily, patients weighing 17 kg to less than 35 kg can take 200 mg once daily, and patients weighing less than 17 kg can take 150 mg daily with or without food.

The most common side effects of Sofosbuvir tablets along with ribavirin are fatigue and headache, and along with peginterferon alfa and ribavirin are nausea, fatigue, insomnia, headache, and anemia.

Warning and Precautions

  • HBV reactivation may occur in HCV/HBV coinfected patients who are undergoing or finished HCV direct-acting antiviral treatment, and those who are not taking hepatitis B virus (HBV) antiviral treatment.
  • Patients need to be tested for evidence of current or known HBV infection by measuring HBsAg and anti-HBc before beginning HCV therapy with Sofosbuvir 400 mg pills. In those with serologic existence of hepatitis B virus infection, assess for clinical and laboratory signs of hepatitis flare or hepatitis B virus reactivation during therapy and during post-therapy follow-up. Initiate relevant patient management for hepatitis B virus infection.
  • Patients who are taking SoviHep who need to initiate amiodarone therapy because of no other substitute, or viable therapy options must undergo cardiac monitoring. Because of amiodarone’s long half-life, patients stopping amiodarone just before initiating SoviHep should also undergo cardiac monitoring.
  • Medicines that are P-gp inducers in the intestine (St. John’s wort and rifampin) may significantly decrease the plasma concentrations of sofosbuvir and may decrease the therapeutic effect of this medicine. The use of rifampin or/and St. John’s wort with Sofosbuvir is not recommended.
