The most common side effects of Bosentan tablets are headache, respiratory tract infection, low blood pressure, fainting, flushing, inflamed nose passages (sinusitis), joint pain, and irregular heart beats.
Elevations of AST/ALT associated with Bosentan are dose-dependent, occur both early and late in therapy, typically progress gradually, are generally asymptomatic, and usually may be reversible after therapy interruption or cessation. Aminotransferase elevations also may reverse spontaneously while continuing this treatment.
Measure liver aminotransferase levels prior to initiation of Bosentan therapy and then monthly and treatment adjusted accordingly. Discontinue treatment if liver aminotransferase elevations are accompanied by clinical signs and symptoms of hepatotoxicity (such as nausea, abdominal pain, vomiting, fever, jaundice, or unusual lethargy or fatigue).
Therapy with Bosentan 62.5 mg/125 mg pills can cause a dose-related reduction in hemoglobin and hematocrit. Check hemoglobin concentrations after 1 and 3 months, and every 3 months thereafter. If a marked reduction in hemoglobin concentration occurs, further evaluation must be undertaken to specify the cause and requirement for specific treatment.
Obtain a pregnancy test in females of reproductive age/potential prior to Bosenat treatment, monthly throughout treatment and one month after stopping therapy. Initiate treatment with Bosentan in females of reproductive age/potential only after a negative pregnancy test.
