Human Normal Immunoglobulin is used to treat the following conditions:

  • Patients with primary hematopoietic immunodeficiency (PI) who are two years of age or older. This encompasses several conditions such as congenital agammaglobulinemia, X-linked agammaglobulinemia, Wiskott-Aldrich syndrome, severe combination immunodeficiencies, and common variable immunodeficiency.

  • Idiopathic Thrombocytopenic Purpura (ITP): to increase platelet counts to stop bleeding or to enable surgery for a patient with ITP. 

  • The goals of maintenance therapy for Chronic Inflammatory Demyelinating Polyneuropathy (CIDP) are to reduce neuromuscular dysfunction and disability and to avoid relapses.

Dosage and Side Effects

Primary Humoral Immunodeficiency (PI):

  • Dosage: 300-600 mg per kg body weight every 3-4 weeks. Infusion starts slow, increasing to 8 mg/kg/min max. Adjustments are based on response.

  • Side effects may include headache, nausea, cough, injection site reaction, pharyngitis, and urticaria.

Idiopathic Thrombocytopenic Purpura (ITP):

  • Dosage: 2 g/kg given in 2 doses over 2 days or in 5 doses over 5 days. Rate starts slow, up to 8 mg/kg/min max. Adjust based on platelet count.

  • Side effects may include headache, fever, vomiting, nausea, back pain, and rash.

Chronic Inflammatory Demyelinating Polyneuropathy (CIDP):

  • Dosage: Loading dose of 2 g/kg over 2-4 days, then maintenance doses every 3 weeks. Start slow, increase to 8 mg/kg/min max. Adjust based on response.

  • Side effects may include headache, chills, hypertension, fever, rash, nausea, and asthenia.

Warning and Precautions

  • Injection of Globucel is not recommended for anyone who has experienced a severe systemic reaction or anaphylaxis following human immune globulin delivery.

  • Patients with IgA deficiency, antibodies against IgA, and a history of known or prior hypersensitivity are not allowed to use this medication.

  • Globucel is one of the IGIV products that can cause severe hypersensitivity responses. In the event of hypersensitivity, stop the infusion right away and start the proper care.

  • When using IGIV products, especially those that contain sucrose, side effects such as acute renal failure, acute tubular necrosis, osmotic nephrosis, proximal tubular nephropathy, and even death are possible. Urine production and renal function must be periodically monitored.

  • When getting IGIV medication, which includes Globucel 10gm/100mL injection, patients may experience hyperproteinemia, hyponatremia, and increased serum viscosity.

  • Rarely, after receiving IGIV therapy—including Globucel—AMS may manifest. When IGIV treatment is stopped, AMS has gone into remission in a few of days without any side effects.

  • Globucel and other IGIV products may contain blood group antibodies that function as hemolysins and cause RBCs to be coated with immunoglobulin in vivo, which can result in a positive direct antiglobulin reaction and, in rare cases, hemolysis. Examine patients for hemolysis symptoms or clinical indicators.
