Description

Arsenic trioxide is an arsenical that falls under the general group of medicines called antineoplastics. It is used in combination with the drug tretinoin for the treatment of adults with newly diagnosed low-risk acute promyelocytic leukemia (APL: cancer of the blood and bone marrow) whose APL is characterized by the existence of the t(15;17) translocation or PML/RAR-alpha gene expression.

The medicine Arsenic trioxide is also used for induction of remission and consolidation in patients with acute promyelocytic leukemia (APL: a form of leukemia) who are refractory to, or have relapsed from, retinoid and anthracycline chemotherapy, and whose disease is characterized by the existence of the t(15;17) translocation or PML/RAR-alpha gene expression.

Dosage and Side Effects:

Newly-diagnosed low-risk APL: 

  • Induction: The recommended dose is 0.15 mg/kg/day intravenously daily in combination with medicine tretinoin until bone marrow remission. Avoid exceeding 60 days.
  • Consolidation: The recommended dose is 0.15 mg/kg/day intravenously (IV) daily for 5 days per week during weeks 1 to 4 of each 8-week cycle for a total of four cycles along with the drug tretinoin. 

Relapsed or refractory APL:

  • Induction: The recommended dose is 0.15 mg/kg/day intravenously daily until bone marrow remission. Avoid exceeding 60 days.
  • Consolidation: The recommended dose is 0.15 mg/kg/day intravenously daily for 25 doses over a period of up to 5 weeks.

 The most common side effects of Arsenic trioxide are abdominal pain, nausea, fatigue, pyrexia, dyspnea, headache, vomiting, tachycardia, diarrhea, hypokalemia, leukocytosis, hyperglycemia, QTc prolongation, cough, hypomagnesemia, insomnia, sore throat, dermatitis, edema, rigors, arthralgia, paresthesia, and pruritus.

Warning and Precautions:

  • Differentiation syndrome (life-threatening or fatal) may occur in those with acute promyelocytic leukemia (APL) treated with Arsenic trioxide. If it is suspected, temporarily withhold treatment and promptly start dexamethasone 10 mg intravenously (IV) every 12 hours and hemodynamic monitoring until elimination of signs and symptoms for at least three days.
  • Patients treated with this drug can develop torsade de pointes, QTc prolongation, and complete atrioventricular block. Before initiating treatment, assess the QTc interval by electrocardiogram, correct pre-existing electrolyte abnormalities, and consider discontinuing medications known to prolong QTc interval. 
  • Severe encephalopathies may occur in patients receiving Arsenic trioxide. Monitor patients for neurological symptoms, such as seizures, confusion, decreased level of consciousness, cognitive deficits, ataxia, visual symptoms, and ocular motor dysfunction. Instruct patients and caregivers on the requirement for precise observation.
  • Wernicke’s encephalopathy may occur in patients receiving this medicine. Monitor patients for nutritional status and neurological symptoms while receiving it. If Wernicke’s encephalopathy is suspected, promptly interrupt treatment and start parenteral thiamine. Monitor until symptoms are eliminated or enhanced and thiamine levels normalize.
  • Those with newly diagnosed low-risk APL treated with Arsenic along with tretinoin experienced alkaline phosphatase, elevated aspartate aminotransferase (AST), and/or serum bilirubin. During treatment, monitor hepatic function tests at least twice a week during induction and at least once a week during consolidation.
  • The active pharmaceutical ingredient (API) of Arsenox, arsenic trioxide, is a human carcinogen. Monitor respective patients for the establishment of second primary malignancies. 
  • This medicinal product can be responsible for causing fetal harm if administered to a pregnant woman. The respective females of reproductive potential should use apt contraception during treatment and for six months after the final dose.
