Lenvatinib Capsule is a kinase inhibitor that is used for the treatment of 

  • Differentiated Thyroid Cancer (DTC): patients with locally recurrent or metastatic, progressive, radioactive iodine-refractory differentiated thyroid cancer (DTC). 
  • Renal Cell Carcinoma (RCC): In conjunction with pembrolizumab, for the first line therapy of adults with advanced renal cell carcinoma (RCC). In conjunction with medicine Everolimus, for adults with advanced renal cell carcinoma (RCC) after one previous anti-angiogenic therapy.
  • Hepatocellular Carcinoma (HCC): For patients with unresectable hepatocellular carcinoma (HCC). 
  • Endometrial Carcinoma (EC): In conjunction with pembrolizumab, for patients with advanced endometrial carcinoma (EC) that is mismatch repair proficient (pMMR), or not microsatellite instability-high (MSI-H), who have disease progression after previous systemic therapy in any setting and are not recipients for curative surgery or radiation.

Dosage and Side Effects

Recommended Dosage for Differentiated Thyroid Cancer (DTC): The recommended dosage of Lenvatinib is 24 mg orally once daily until there is evidence of disease progression or until unacceptable toxicity. 

Recommended Dosage for Renal Cell Carcinoma (RCC): The recommended dosage of Lenvatinib for first-line treatment of patients with advanced RCC is 20 mg orally once daily along with pembrolizumab 200 mg administered as an intravenous (IV) infusion over 30 minutes every 3 weeks until there is evidence of disease progression or unacceptable toxicity or up to 2 years.

The recommended dosage of Lenvatinib for previously treated RCC is 18 mg along with everolimus 5 mg orally once daily until there is evidence of disease progression or unacceptable toxicity. 

Recommended Dosage for Hepatocellular Carcinoma (HCC): The recommended dosage of Lenvatinib is 12 mg for patients greater than or equal to 60 kg or 8 mg for patients less than 60 kg. Take it orally once daily until there is evidence of disease progression or unacceptable toxicity.

Recommended Dosage for Endometrial Carcinoma (EC): The recommended dosage of Lenvatinib is 20 mg orally once daily, along with pembrolizumab 200 mg administered as an IV infusion over 30 minutes every 3 weeks, until there is evidence of unacceptable toxicity or disease progression.

Take Lenvatinib dosage once daily, with or without food, at the same time each day. If a dose is missed and cannot be taken within 12 hours, skip that dose of Lenvatinib and take the next dose at the usual time of administration.

The most common side effects of Lenvatinib in Differentiated Thyroid Cancer (DTC), Renal Cell Carcinoma (RCC), Hepatocellular Carcinoma (HCC), and Endometrial Carcinoma (EC) are as:

  • DTC: tiredness, nausea, weight loss, vomiting, joint and muscle pain, decreased appetite, mouth sores, rash, redness, headache, abdomen pain, itching, and hoarseness.
  • RCC: tiredness, mouth sores, joint and muscle pain, decreased appetite, nausea, swelling in your arms and legs, vomiting, cough, abdomen pain, trouble breathing, weight loss, bleeding, and rash.
  • HCC: hypertension, decreased appetite, fatigue, diarrhea, arthralgia/myalgia, decreased weight, abdominal pain, palmar-plantar erythrodysesthesia syndrome, hypothyroidism, proteinuria, dysphonia, hemorrhagic events, and nausea.
  • EC: fatigue, diarrhea, hypothyroidism, hypertension, musculoskeletal disorders, decreased appetite, vomiting, hemorrhagic events, stomatitis, decreased weight, urinary tract infection, abdominal pain, proteinuria, constipation, headache, palmar-plantar erythrodysesthesia, rash, dysphonia, and nausea.

Warning and Precautions

  • Serious cardiac dysfunction can occur with Lenvatinib. Monitor patients for clinical symptoms or signs of cardiac dysfunction. Withhold and resume treatment at a reduced dose upon recovery or permanently stop Lenvatinib based on severity.
  • Arterial thromboembolic events of any severity may occur. Permanently stop Lenvatinib 4 mg/10 mg treatment after an arterial thrombotic event. The safety of resuming Lenvatinib treatment after an arterial thromboembolic event has not been established.
  • Serious hepatic adverse reactions may occur in patients treated with Lenvatinib. Monitor liver function before starting Lenvatinib treatment, then every 2 weeks for the initial 2 months, and at least monthly thereafter throughout treatment. Monitor patients with HCC Precisely for signs of hepatic failure, including hepatic encephalopathy.
  • Serious renal failure or impairment can occur with Lenvatinib 10 mg/4 mg treatment. Start prompt management of diarrhea or dehydration/hypovolemia. Withhold and resume therapy at a reduced dose upon recovery or permanently stop Lenvatinib for renal failure or impairment on behalf of severity.
  • Proteinuria may occur in patients treated with Lenvatinib. Monitor them for proteinuria before initiating Lenvatinib and periodically during treatment. In case urine dipstick proteinuria higher than or equal to 2+ is detected, obtain a 24-hour urine protein.
