Atracurium besylate, also called atracurium besilate is a highly selective, competitive or non-depolarising neuromuscular blocking agent which is used as an adjunct/addition to general anaesthesia to enable tracheal intubation to be done and to relax skeletal muscles during surgery or controlled ventilation, and to facilitate mechanical ventilation in Intensive Care Unit (ICU) patients. 

Dosage and Side Effects

The recommended dosage of Atracurium besylate injection is 0.23 mg/kg and 0.5 mg/kg for adults and children older than 2 years of age, 0.3-0.4 mg/kg for children less than 2 years of age. In morbidly obese patients, the dose should be administered on behalf of the ideal body weight.

The most common side effects of atracurium besylate 25 mg/2.5 mL injection are flushing and erythema.

Warning and Precautions

  • In common with all the existing neuromuscular blocking agents monitoring of neuromuscular function is recommended during the therapy with Atracurium besylate in order to individualize dosage requirements.

  • Extreme caution must be exercised in administering atracurium 25 mg/2.5 mL to those in whom substantial histamine release would be hazardous (such as patients with clinically significant cardiovascular complications) and in patients with any history (such as critical anaphylactoid reactions or asthma) suggesting a higher risk of histamine release.

  • Since atracurium injection has no clinically significant impact on heart rate in the recommended dosage range, it will not counteract the bradycardia produced by several anesthetic agents or vagal stimulation. As a result, bradycardia throughout anesthesia may be more common with atracurium injection than with other existing muscle relaxants.

  • Atracurium therapy may have profound impacts on those with myasthenia gravis, Eaton-Lambert syndrome, or other neuromuscular complications in which potentiation of nondepolarizing agents has been observed. The use of a peripheral nerve stimulator is important for assessing neuromuscular block in such patients. Precautions must be considered in those with severe electrolyte disorders or carcinomatosis.

  • Use atracurium injection during pregnancy only in case the potential benefit justifies the risk to the fetus. It is unknown whether this medicine is excreted in human milk. Because many other medicines are excreted in human milk, caution must be exercised when it is administered to a nursing woman.
