Description

A nucleoside analogue of CMV DNA polymerase inhibitor, ganciclovir is used for the following:

  • Adult immunocompromised patients, especially those with acquired immunodeficiency syndrome (AIDS), are treated for CMV retinitis.
  • prevention of CMV illness in adult transplant recipients who are susceptible to the illness

Dosage and Side Effects

Avoid administering the suggested dose of ganciclovir injection via rapid or direct intravenous injection, as this could lead to increased toxicity due to higher plasma levels. Always accompany the injection with sufficient hydration.

For treating CMV Retinitis in adults: Initial treatment involves a Ganciclovir dosage of 5 mg/kg, given intravenously over one hour every 12 hours for 14-21 days. After this induction phase, the recommended maintenance dose for CMV Retinitis patients is 5 mg/kg, administered intravenously over one hour, once daily for 7 days weekly. Alternatively, a dosage of 6 mg/kg once daily for 5 days weekly may be considered.

For preventing CMV Disease in adult transplant recipients: The suggested initial dosage of ganciclovir injection for patients with normal renal function is 5 mg/kg, administered intravenously over one hour every 12 hours for 7-14 days. Following the induction phase, the maintenance dose of Ganciclovir is recommended at 5 mg/kg, given intravenously over one hour, once daily for 7 days weekly. Another option is 6 mg/kg once daily for 5 days weekly until 100-120 days post-transplantation.

Frequent side effects of Ganciclovir include fever, leukopenia, diarrhea, anemia, events related to catheter use such as infection and sepsis, as well as increased levels of creatinine.

Warning and Precautions

  • Patients who have had clinically significant hypersensitivity responses (such as anaphylaxis) to ganciclovir, valganciclovir, or acyclovir are not eligible to receive 500 mg of ganciclovir.

  • Frequent complete blood counts, along with differential and platelet counts, are necessary, especially for patients whose absolute neutrophil counts at the beginning of treatment are less than 1000 cells/μL.

  • Prior to and during ganciclovir 500 mg injectable treatment, patients should have their renal function monitored. If necessary, the dosage should be adjusted.

  • While receiving ganciclovir medication, patients with CMV retinitis should undergo routine ophthalmological examinations to check for other retinal abnormalities and to monitor the disease’s progress.

  • Pregnancy tests should be performed on females who are or may become pregnant before starting Ganciclovir medication.

  • Because ganciclovir 500 mg has the potential to produce serious adverse events in nursing infants and because HIV can be transmitted to the baby through breast milk, nursing women should not nurse during therapy.
