Trientine is a chelating agent, used for the treatment of Wilson’s disease (hepatolenticular degeneration) in patients intolerant to medication known as Penicillamine. The therapeutic drug should be used when continued treatment with Penicillamine is no longer possible because of intolerable or life-endangering side effects.

Limitation(s) of use: Unlike penicillamine, the medication Trientine is not recommended in cystinuria or rheumatoid arthritis (RA). Trientine is not indicated to treat biliary cirrhosis.

Dosage and Side Effects

The recommended initial dosage is 500-750 mg per day for pediatric patients and 750-1,250 mg per day for adults given in divided doses 2, 3, or 4 times daily. Dosage may be enhanced to a maximum of 2,000 mg daily for adults or 1,500 mg daily for pediatric patients aged 12 or less.

The daily dosage of Trientine capsules needs to be enhanced only if the clinical response (CR) is not apt or the free serum copper concentration is constantly beyond 20 mcg/dL.

Take Trientine dosage on an empty stomach, at least 60 minutes prior to a meal or 120 minutes after a meal, and at least 60 minutes apart from any other medicine, food, or milk. Swallow the capsules whole through water and it must not be chewed or opened.

The most common side effects of trientine hydrochloride may include heartburn, stomach pain, loss of appetite, mouth sores, general ill feeling, black/tarry stools, skin flaking, cracking, or thickening.

Warning and Precautions

  • Trientine therapy is not recommended if patients are allergic to it. Patients must remain under regular medical supervision throughout the therapy. Clinicians should regularly check to see if patients have iron deficiency anemia. This is mainly required for women. A clinician should also check for free copper in the blood.

  • Trientine is a chelating compound that reduces the serum iron levels. Iron supplementation may be needed in a few cases. Concomitant oral iron needs to be considered at a different time than trientine.

  • The concomitant use of trientine 250 mg capsules and zinc is not recommended. There are only limited findings/data on concomitant use available and no specific dosage recommendations can be considered. 

  • In patients who have previously been treated with D-Penicillamine, lupus-like reactions have been seen throughout subsequent therapy with trientine capsule, however, it is not possible to specify in case there is a causal bond with trientine.

  • Patients with renal impairment or hepatic impairment taking treatment with trientine capsules should remain under regular and precise medical supervision for apt control of symptoms and copper levels. Precise monitoring of renal or liver function is also required in these populations.
