Description

Liposomal Amphotericin B is a treatment of invasive fungal infection in patients who are refractory to or intolerant of conventional amphotericin B therapy. This antifungal and an antiprotozoal medicine is used for:

  • empirical therapy for presumed fungal infection in febrile, neutropenic patients

  • treatment of Cryptococcal Meningitis in HIV-infected patients

  • treatment of patients with Aspergillus species, Candida species, and/or Cryptococcus species infections refractory to amphotericin B deoxycholate, or in patients where renal impairment or unacceptable toxicity precludes the use of amphotericin B deoxycholate

  • treatment of visceral leishmaniasis. In immunocompromised individuals with visceral leishmaniasis treated with Liposomal Amphotericin B, relapse rates were high after initial clearance of parasites.

Dosage and Side Effects

The recommended dosage of Liposomal Amphotericin B should be administered by intravenous infusion, through a controlled infusion device, over a time-span of approximately 120 minutes. The recommended initial dose for each indication for adults and pediatric patients is as follows:

  • For Empirical therapy: The recommended dose is 3 mg/kg/day.

  • For Systemic fungal infections (Aspergillus, Candida, Cryptococcus): The recommended dose is 3 to 5 mg/kg/day.

  • Cryptococcal meningitis in HIV infected patients: The recommended dose is 6 mg/kg/day.

Standard doses of injection of liposomal amphotericin b for visceral leishmaniasis are as follows:

  • For Immunocompetent patients: The recommended dose is 3 mg/kg/day (days 1 to 5) and 3 mg/kg/day on days 14, 21.

  • For Immunocompromised patients: Recommended dose is 4 mg/kg/day (days 1 to 5) and 4 mg/kg/day on days 10, 17, 24, 31, 38.

The most common side effects of Liposomal Amphotericin B are hyperglycemia, hypokalemia, nausea, rash, diarrhea, vomiting, anemia, increased alkaline phosphatase, increased creatinine, increased blood urea nitrogen, hypomagnesemia, chills, insomnia, and dyspnea.

Warning and Precautions

  • Anaphylaxis may occur with amphotericin B deoxycholate and other existing amphotericin B-containing therapeutic drugs, including liposomal amphotericin b injection. If a critical anaphylactic reaction occurs, the infusion needs to be promptly stopped and the patient should not receive further infusions.

  • As with any amphotericin B-containing medicinal product the medicine should be administered by a professionally trained personnel. During the initial dosing period, patients should be under close clinical observation. This medicinal product is significantly less toxic than amphotericin B deoxycholate; however, some side effects may still emerge

  • Patients receiving treatment with this medicine should be monitored for serum potassium, urea, calcium, urine output, magnesium, creatinine, liver function tests, and complete blood count.

  • Renal toxicity basically is dose dependent; dose adjustments need to be considered only after evaluating and considering the patient’s overall clinical picture.
