Amphotericin B Liposomal Injection is an antifungal and an antiprotozoal medicine used for:

  • Treatment of Cryptococcal Meningitis in HIV infected patients.
  • Empirical therapy for presumed fungal infection in febrile, neutropenic patients.
  • Treatment of visceral leishmaniasis
  • Treatment of patients with Aspergillus species, Candida species and/or Cryptococcus species infections refractory to amphotericin B deoxycholate, or in patients where renal impairment or unacceptable toxicity precludes the use of amphotericin B deoxycholate.

Dosage and Side Effects

Dosage of Liposomal Amphotericin B should be administered by intravenous infusion, using a controlled infusion device, over a time period of approximately 120 minutes.

The recommended initial dose of liposomal amphotericin b 50 mg injection for each indication for adults and pediatric patients is as follows:

  • For Empirical therapy: Dose is 3 mg/kg/day.
  • For Systemic fungal infections (Aspergillus, Candida, Cryptococcus): Dose is 3-5 mg/kg/day.
  • Cryptococcal meningitis in HIV infected patients: Dose is 6 mg/kg/day.

Standard doses of injection of liposomal amphotericin b for visceral leishmaniasis are as follows:

  • For Immunocompetent patients: Dose is 3 mg/kg/day (days 1-5) and 3 mg/kg/day on days 14, 21.
  • For Immunocompromised patients: Dose is 4 mg/kg/day (days 1-5) and 4 mg/kg/day on days 10, 17, 24, 31, 38.

The most commonly reported side effects due to liposomal amphotericin b injection include: fever, chills, emesis, nephrotoxicity, hypokalemia, increased serum creatinine, and infusion related adverse events.

Warning and Precautions

  • As with any amphotericin B-containing product the medicine should be given by a medically trained personnel. During the first dosing period, patients should be under precise clinical observation. This drug is significantly less toxic than amphotericin B deoxycholate; however, side effects may still occur.
  • Anaphylaxis may appear with amphotericin B deoxycholate and other existing amphotericin B-containing medicines, including liposomal amphotericin injection. In case a severe anaphylactic reaction emerges, the infusion needs to be promptly discontinued and the patient should not receive further infusions.
  • Patients receiving treatment with this medicinal product should be monitored for serum potassium, calcium, magnesium, creatinine, urea, urine output, liver function tests and CBC.
  • Patient management needs to include laboratory assessment of hepatic, renal, and hematopoietic function, and serum electrolytes (particularly potassium and magnesium).
  • Renal toxicity basically is dose dependent; dose adjustments need to be done only after assessing and considering the patient’s overall clinical picture.
