Description

Ibrutinib is an orally administered, selective and covalent inhibitor of the enzyme called Bruton’s tyrosine kinase (BTK). This oral therapeutic medicine is used for the management and treatment of:

  • adult patients with mantle cell lymphoma (MCL) who have previously been treated with at least one therapy (This indication of Ibrutinib is approved under accelerated approval on behalf of the overall response rate).
  • adult individuals with chronic lymphocytic leukemia-CLL/Small lymphocytic lymphoma-SLL.
  • adult patients with chronic lymphocytic leukemia-CLL/Small lymphocytic lymphoma-SLL with 17p deletion.
  • adult patients with Waldenström’s macroglobulinemia (WM).
  • adult patients with marginal zone lymphoma (MZL) who require systemic therapy and have been treated with at least one prior anti-CD20-based treatment (This indication of Ibrutinib 140 mg is approved under accelerated approval on behalf of the overall response rate).
  • adult and pediatric patients aged 1 year and older with chronic graft versus host disease (cGVHD) after failure of single or multiple lines of systemic therapy.

Dosage and Side Effects

Mantle Cell Lymphoma (MCL) and Marginal Zone Lymphoma (MZL): The recommended dosage for MCL and MZL is 560 mg administered orally once daily until disease progression or unacceptable toxicity.

Chronic Lymphocytic Leukemia (CLL)/Small Lymphocytic Lymphoma (SLL) and Waldenström’s Macroglobulinemia (WM): The recommended dosage for CLL/SLL and WM is 420 mg orally once daily until disease progression or unacceptable toxicity. For CLL/SLL, Ibrutinib can be used as a single agent, in combination with medicine rituximab or obinutuzumab, or together with bendamustine and rituximab (BR). For WM, Ibrutinib can be used as a single agent or together with rituximab. 

Chronic Graft versus Host Disease (cGVHD): The recommended dosage for patients aged 12 years and older with cGVHD is 420 mg administered orally once daily, and for patients aged 1 to less than 12 years with cGVHD is 240 mg/m2 given orally once daily (up to a dose of 420 mg), until disease progression, or recurrence of an underlying malignancy, or unacceptable toxicity. Swallow capsules completely through a glass of water. Avoid chewing, opening, or breaking the capsules.

The most common side effects of Ibrutinib in patients with B-cell malignancies (MCL, CLL/SLL, WM, and MZL) are diarrhea, thrombocytopenia, fatigue, musculoskeletal pain, neutropenia, anemia, bruising, and rash.

The most common adverse reactions in adult or pediatric patients with cGVHD are musculoskeletal pain, fatigue, anemia, diarrhea, bruising, thrombocytopenia, pyrexia, stomatitis, muscle spasms, hemorrhage, nausea, headache, abdominal pain, and pneumonia.

Warning and Precautions

  • Fatal bleeding events may occur in patients who received Ibrutinib. Use of either antiplatelet or anticoagulant agents concomitantly with Ibrutinib may increase the risk of major hemorrhage. Consider the risks and benefits of antiplatelet or anticoagulant therapy when co-administered with Ibrutinib. Monitor patients for bleeding-associated signs and symptoms. 
  • Fatal and non-fatal infections (including viral, bacterial, or fungal) may occur with Ibrunat 140 mg treatment. Monitor and assess all patients for fever and infections and treat them appropriately.  
  • Fatal and critical cardiac arrhythmias and cardiac failure may occur with this medicine. Evaluate cardiac history and function at baseline, and assess patients for cardiac arrhythmias and cardiac function. Obtain further evaluation (such as ECG, or echocardiogram) as required.
  • Hypertension may develop in patients taking treatment with Ibrutinib. Monitor blood pressure in those treated with Ibrutinib oral drug, initiate or adjust anti-hypertensive medication throughout the treatment as appropriate, and follow dosage modification guidelines for Grade 3 or higher hypertension.
  • Tumor lysis syndrome (TLS) may infrequently develop with Ibrutinib therapy. Assess the baseline risk (such as high tumor burden) and take all the relevant precautions. Monitor patients precisely and treat them as appropriate.
  • In patients with B-cell malignancies who are receiving treatment with Ibrutinib as a single agent, grade 3/4 neutropenia, grade 3/4 thrombocytopenia, and grade 3/4 anemia may occur, based on laboratory measurements. Monitor complete blood counts (CBC) monthly.
