Atgam 250 mg Injection (Horse anti-human T-lymphocyte immunoglobulin) is a hyperimmune globulin preparation made from the plasma of horses and immunized with human thymocytes or T cells. This immunoglobulin G is used for renal allograft rejection and aplastic anemia (moderate to severe) in individuals not suitable for bone marrow transplantation (BMT).

Limitations of Use: The usefulness of Atgam 250 mg has not been shown in those with aplastic anemia who are suitable candidates for BMT or in patients with aplastic anemia secondary to neoplastic disease, storage disease, myelofibrosis, Fanconi’s syndrome, or in individuals known to have been exposed to myelotoxic agents or radiation.

Dosage and Side Effects

Renal Allograft Rejection: The recommended dosage of ATGAM injection for renal allograft rejection is 10-15 mg/kg daily intravenously for 2 weeks (14 days). Additional alternate-day therapy up to a total of 21 doses may be considered.

Aplastic Anemia (Moderate to Severe): The recommended dosage for aplastic anemia is 10 to 20 mg/kg daily administered intravenously for 8-14 days. Additional alternate-day therapy up to a total of 21 doses may be considered.

The most common side effects of ATGAM are chills, pyrexia, rash, thrombocytopenia, leukopenia, and arthralgia.

Warning and Precautions

  • Cytokine release syndrome may occur with the use of ATGAM injection, which can be fatal. Signs and symptoms may include fever, chills, headache, chest pain, hypotension, dyspnea, tachypnea, and edema. Assess patients for signs and symptoms of cytokine release syndrome and manage them as per clinical guidelines.

  • Anaphylaxis may occur with the use of ATGAM. Anaphylaxis is life-threatening. Stop treatment in case anaphylaxis develops. Rash, dyspnea, tachycardia, and hypotension can be signs/symptoms of an anaphylactic reaction. Assess patients for symptoms of anaphylaxis during infusion and for at least 24 hours after the infusion.

  • Critical infusion-related reactions may occur with the administration of ATGAM 250 mg/5 mL injection. Signs and symptoms associated with infusion-associated reactions include dyspnea, tachycardia, rash, and hypotension. Assess patients for infusion-associated reactions and manage them as per clinical guidelines.

  • Serum sickness is a resisted hypersensitivity or immune reaction. Signs and symptoms of serum sickness may include arthralgia, pyrexia, chills, rash, and pain. Assess all the patients for serum sickness and manage them as per clinical guidelines.

  • Due to the immunosuppressive effects of ATGAM/ATG, opportunistic infections (such as viral, bacterial, and fungal) may occur. Sepsis may also occur. There is an increased probability of viral reactivation (such as cytomegalovirus, Epstein–Barr virus infection, and herpes simplex virus). Assess all the patients for concurrent infection. 

  • Thrombocytopenia and neutropenia may occur with the use of ATG or ATGAM injection. In case thrombocytopenia occurs, consider platelet transfusions to maintain platelets at required levels. Stop treatment in case serious and unremitting thrombocytopenia or neutropenia occurs.
