Description

Aztreonam is a beta-lactam antibiotic agent used for the treatment of the following infections caused by susceptible Gram-negative microorganisms:

  • Urinary Tract Infections (complicated/uncomplicated), including pyelonephritis and cystitis (initial/recurrent) caused by Escherichia coli, Proteus mirabilis, Pseudomonas aeruginosa, Klebsiella pneumoniae, Enterobacter cloacae, Citrobacter species, Klebsiella oxytoca, and Serratia marcescens.

  • Lower Respiratory Tract Infections, including pneumonia and bronchitis caused by Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, Haemophilus influenzae, Enterobacter species, Proteus mirabilis, and Serratia marcescens. 

  • Septicemia caused by Klebsiella pneumoniae, Escherichia coli, Pseudomonas aeruginosa, Proteus mirabilis, Enterobacter species, and Serratia marcescens.

  • Skin and Skin-Structure Infections, including those associated with postoperative wounds, ulcers, and burns, caused by Proteus mirabilis, Escherichia coli,, Enterobacter species, Pseudomonas aeruginosa, Serratia marcescens, Citrobacter species, and Klebsiella pneumoniae.

  • Intra-abdominal Infections, including peritonitis caused by Escherichia coli, Klebsiella species including K. pneumoniae, Enterobacter species including E. cloacae, Pseudomonas aeruginosa, Citrobacter species including C. freundii, and Serratia species including S. marcescens.

  • Gynecologic Infections, including endometritis and pelvic cellulitis caused by Escherichia coli, Klebsiella pneumoniae, Enterobacter species including E. cloacae, and Proteus mirabilis. 

Aztreonam is used for adjunctive therapy to surgery for managing infections caused by susceptible organisms, including abscesses, infections complicating hollow viscus perforations, cutaneous infections, and infections of serous surfaces. Aztreonam is promising against certain most commonly encountered Gram-negative aerobic pathogens observed in general surgery.

Dosage and Side Effects

Aztreonam may be administered either intravenously or by intramuscular injection. Dosage and route of administration need to be determined by susceptibility of the causative organisms, severity and site of infection, and the current condition of the patient.

The recommended dosage for patients with Urinary tract infections is 500 mg or 1 g administered every 8 or 12 hours, for patients with moderately severe systemic infections is 1 g or 2 g administered every 8 or 12 hours, and for patients with severe systemic or life-threatening infections is 2 g administered every 6 or 8 hours.

Because of the critical nature of infections due to Pseudomonas aeruginosa, the dosage of 2 g every 6 or 8 hours is recommended, at least upon starting therapy, in systemic infections caused by this organism.

The intravenous (IV) route is recommended for those requiring single doses higher than 1 g or those with bacterial septicemia, localized parenchymal abscess, peritonitis, or other severe systemic or serious infections.

The duration of treatment depends on the severity of infection. The maximum recommended dosage is 8 g per day.

The most common side effects of Aztreonam are fever, chest discomfort, cough, difficulty with breathing or troubled breathing.

Warning and Precautions

  • Prescribing Aztreonam in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to offer benefit to the patient and increases the probability of the establishment of drug-resistant bacteria. 

  • In case an aminoglycoside is used along with aztreonam, mainly if high dosages of the former are used or if treatment is prolonged, renal function needs to be monitored because of the potential nephrotoxicity and ototoxicity of aminoglycoside antibiotics.

  • The use of antibiotics may be responsible for promoting the overgrowth of nonsusceptible organisms, including Gram-positive organisms (Staphylococcus aureus and Streptococcus faecalis) and fungi. Should superinfection occur throughout therapy, apt measures should be considered.

  • In patients with impaired hepatic or renal function, appropriate monitoring is recommended throughout the therapy with Aztreonam. 

  • Patients need to be counseled that antibacterial medicines including Aztreonam should only be used for treating bacterial infections. They do not treat viral infections (such as the common cold).
