Treosulfan 5 mg Injection in combination with fludarabine is used as part of conditioning treatment before allogeneic hematopoietic stem cell transplantation (alloHSCT) in adults and in pediatric patients older than one month with malignant and non-malignant diseases. 

Dosage and Side Effects

Adults with malignant disease: Treosulfan Injection is given along with fludarabine. The recommended dosage is 10 g/m² body surface area (BSA) per day as a two-hour intravenous infusion, given on three consecutive days (day -4, -3, -2) before stem cell infusion (day 0). The total treosulfan injection dose is 30 g/m². Fludarabine 30 mg/m² BSA daily as a 0.5-hour intravenous (IV) infusion, given on five consecutive days (day -6, -5, -4, -3, -2) before stem cell infusion (day 0). The total fludarabine dose is 150 mg/m². Treosulfan needs to be given before fludarabine on days -4, -3, -2.

Adults with non-malignant disease: Treosulfan injection is given in combination with fludarabine with or without thiotepa. The recommended dose is 14 g/m² body surface area (BSA) per day as a two-hour intravenous infusion, given on 3 consecutive days (day -6, -5, -4) before stem cell infusion (day 0). The total treosulfan dose is 42 g/m². Fludarabine 30 mg/m² BSA daily as a 0.5-hour intravenous (IV) infusion, given on five consecutive days (day -7, -6, -5, -4, -3) before stem cell infusion (day 0). The total fludarabine dose is 150 mg/m². Treosulfan needs to be given before fludarabine on days -6, -5, -4. Thiotepa 5 mg/kg twice daily, given as two intravenous (IV) infusions over 2 to 4 hours on day -2 before stem cell infusion (day 0). 

The most common side effects in adults and children with Treosulfan are infections, nausea (feeling sick), stomatitis (inflammation of the lining of the mouth), vomiting, diarrhea, tiredness, febrile neutropenia (low white blood cell counts with fever), high blood levels of bilirubin (a breakdown product of red blood cells), abdominal pain (belly ache), itching, and fever.

Warning and Precautions

  • Profound myelosuppression with pancytopenia is the desired therapeutic effect of treosulfan, occurring in all the respective patients. Frequently monitor blood cell counts until recovery of the hematopoietic system. 
  • Oral mucositis or OM (including high-grade severity) is an extremely common undesirable effect of treosulfan-based conditioning followed by alloHSCT. Use of mucositis prophylaxis (e.g. barrier protectants, topical antimicrobials, ice, and adequate oral hygiene) is recommended.
  • Treosulfan injection may impair fertility. Therefore, men taking treatment with treosulfan should not father a child during and up to six months after therapy and seek advice on cryo-conservation of sperm before therapy because of the potential of irreversible infertility due to treatment with treosulfan. Ovarian suppression and amenorrhoea with menopausal signs and symptoms occur in pre-menopausal patients.
  • Animal studies are not sufficient with respect to reproductive toxicity. The medicine Treosulfan is contraindicated during pregnancy. It is not specified whether this therapeutic drug is excreted in human milk. Breastfeeding needs to be discontinued while on therapy with treosulfan.
