Description

Carmustine 100 mg injection is a nitrosourea used as palliative therapy as a single agent or in combination therapy with other approved chemotherapeutic agents in the following:

  • Brain tumors glioblastoma, brainstem glioma, astrocytoma, medulloblastoma, ependymoma, and metastatic brain tumors.
  • Multiple myeloma (MM)-in combination with medicine prednisone.
  • Relapsed or refractory Hodgkin’s lymphoma together with other approved medicines.
  • Relapsed or refractory non-Hodgkin’s lymphomas together with other approved therapeutic drugs.

Dosage and Side Effects

The recommended dosage of Carmustine injection as a single agent in previously untreated individuals is 150 mg/m2 to 200 mg/m2 intravenously every 6 weeks. Administer as a single dose or divided into daily injections such as 75 mg/m2 to 100 mg/m2 on two consecutive days. Reduce the dose when Carmustine is used with other myelosuppressive medicines or in patients in whom bone marrow reserve is depleted. Administer Carmustine for the duration as per the established regimen. Premedicate each dose with anti-emetics.  

The most common side effects of Carmustine injection are vomiting, nausea, renal toxicity, pneumonitis, pulmonary toxicity, and myelosuppression.

Warning and Precautions

  • Bone marrow toxicity is a dose-limiting, common, and critical toxic effect of carmustine injection, occurring 4 to 6 weeks following medicine administration. Monitor complete blood count (CBC) weekly for at least 6 weeks after a dose. Repeat doses of Carmustine should not be given more frequently than every 6 weeks.
  • Cases of fatal pulmonary toxicity with carmustine 100 mg treatment may develop. Conduct the baseline pulmonary function studies along with frequent pulmonary function tests throughout treatment.
  • Injection site reactions may occur during the administration of Carmustine. Close monitoring of the infusion site for potential infiltration during drug administration is recommended. A specific treatment for extravasation reactions is not known at this time. 
  • Long-term use of nitrosoureas, such as Carmustine, may cause the development of secondary malignancies. Patients treated with this therapeutic medicine need to be monitored long-term for the development of second malignancies.
  • Carmustine has been administered through an intra-arterial intracarotid route; this procedure is investigational and has been linked with certain ocular toxicity. Safety as well as effectiveness of the intra arterial route have not been established.
