Description

Daclatasvir is a hepatitis C virus (HCV) NS5A inhibitor authorized for use with sofosbuvir, with or without the medicine ribavirin, for the treatment of patients with chronic HCV genotype 1 or 3 infection.

Limitations of Use: Sustained virologic response (SVR12) rates are reduced in genotype 3 patients with cirrhosis receiving Daclatasvir in combination with medicine sofosbuvir for 12 weeks.

Dosage and Side Effects

The recommended dosage of Daclatasvir is 60 mg, taken orally, once daily, with or without food together with sofosbuvir with or without ribavirin. Reduce the dosage of Daclatasvir tablets to 30 mg once daily with strong CYP3A inhibitors and increase the dosage of Daclatasvir to 90 mg once daily with moderate CYP3A inducers. Recommended treatment duration is 12 weeks. 

The most common side effects of Daclatasvir in combination with sofosbuvir are headache and fatigue, and in combination with medicines sofosbuvir and ribavirin are fatigue, headache, anemia, and nausea.

Warning and Precautions

  • Hepatitis B virus (HBV) reactivation may develop in patients coinfected with HCV/HBV who are undergoing or had completed therapy with HCV direct-acting antivirals, and who are not taking HBV antiviral treatment. Test all patients for the presence of current or prior HBV infection by measuring HBsAg and antiHBc prior to starting HCV therapy with Daclatasvir.
  • Cases of symptomatic bradycardia and cases requiring pacemaker intervention may occur when amiodarone is coadministered with medicine sofosbuvir along with another HCV direct-acting antiviral, including Daclatasvir 30 mg/60 mg. Coadministration of medication amiodarone with Daclatasvir along with sofosbuvir is not recommended.
  • Patients with signs or symptoms of bradycardia should seek immediate medical evaluation. Signs and symptoms may include malaise, near-fainting or fainting, dizziness or lightheadedness, chest pain, weakness, shortness of breath, confusion, excessive tiredness, or memory problems.
  • If the medicine sofosbuvir is permanently stopped in a patient receiving Daclatasvir therapy with sofosbuvir, then treatment with Daclatasvir should also be stopped/discontinued.  
  • Consider screening for the existence of NS5A polymorphisms at amino acid positions M28, Q30, L31, and Y93 in those with cirrhosis who are infected with HCV genotype 1a before the initiation of treatment with Daclatasvir tablets and sofosbuvir with or without medicine ribavirin.
