Description

Dasatinib is a targeted therapy medication approved for certain cases of chronic myelogenous leukemia (CML) and acute lymphoblastic leukemia (ALL). This kinase inhibitor is specifically used for the treatment of:

  • Newly diagnosed adults with Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) in the chronic phase (CP).
  • Adults with chronic, accelerated, or myeloid or lymphoid blast phase Ph+ CML with resistance or intolerance to prior therapy including imatinib.
  • Adults with Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukemia (ALL) with resistance or intolerance to prior therapy.
  • Pediatric patients aged 1 year and older with Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) in the chronic phase (CP).
  • Pediatric patients aged 1 year and older with newly diagnosed Ph+ acute lymphoblastic leukemia (ALL) in conjunction with chemotherapy.

Dosage and Side Effects

The recommended starting dosage for chronic phase CML in adults is Dasatinib 100 mg administered orally once daily. The recommended starting dosage for accelerated phase CML, myeloid or lymphoid blast phase CML, or Ph+ ALL in adults is 140 mg administered orally once daily. 

The recommended daily dose for pediatric patients with CML or Ph+ ALL weighing 10 kg to less than 20 kg is 40 mg orally once daily, 20 kg to less than 30 kg is 60 mg orally once daily, 30 kg to less than 45 kg is 70 mg orally once daily, and patients weighing at least 45 kg is 100 mg orally once daily.

Do not chew, crush, or cut tablets. Swallow tablets whole. Dasatinib can be taken with or without food, either in the morning or in the evening.

The most common side effects in patients receiving Dasatinib as single-agent therapy are fluid retention events, myelosuppression, diarrhea, musculoskeletal pain, headache, skin rash, hemorrhage, dyspnea, fatigue, and nausea.

The most common side effects in pediatric patients receiving Dasatinib tablets together with chemo are diarrhea, mucositis, febrile neutropenia, headache, nausea, vomiting, musculoskeletal pain, abdominal pain, pyrexia, cough, constipation, arrhythmia, hypertension, edema, hypotension, decreased appetite, rash, fatigue, dyspnea, hypersensitivity, epistaxis, and altered state of consciousness.

Warning and Precautions

  • In chronic phase CML patients, perform complete blood counts (CBCs) every two weeks for 12 weeks, then every three months thereafter, or as clinically required. In advanced phase CML or Ph+ ALL patients, perform CBCs weekly for the initial two months and then monthly thereafter, or as clinically required. 
  • In Ph+ ALL pediatric patients treated with Dasatinib together with chemotherapy, perform CBCs before the beginning of each block of chemo and as clinically required. During the consolidation blocks of chemo, perform complete blood counts (CBCs) every two days until recovery. 
  • Dasatinib 140 mg/100 mg/80 mg/70 mg/50 mg/20 mg oral tablet can be responsible for causing serious and critical bleeding. Concomitant drugs that inhibit platelet function or anticoagulants may increase the probability of hemorrhage.
  • Dasatinib can cause fluid retention. Assess patients who develop signs and symptoms of pleural effusion or other fluid retention, such as new or worsened dyspnea on exertion or at rest, dry cough, or pleuritic chest pain, quickly with a chest x-ray or additional diagnostic imaging as apt.
  • Dasatinib oral therapy can be responsible for causing cardiac dysfunction. Monitor patients for signs or symptoms consistent with cardiac dysfunction and treat them appropriately.
  • Dasanat 20 mg/50 mg tablets may increase the probability of developing pulmonary arterial hypertension in adult and pediatric patients which may appear any time after initiation, including after beyond 12 months of treatment. Evaluate patients for signs and symptoms of underlying cardiopulmonary disease before starting and during treatment.
