Description

Daratumumab is a CD38-directed cytolytic antibody widely used for the treatment of adult patients with  a type of blood cancer known as multiple myeloma (MM):

  • in combination with drugs lenalidomide (D-Rd) and dexamethasone in newly diagnosed patients who are not eligible for autologous stem cell transplant (ASCT) and in patients with relapsed or refractory multiple myeloma (MM) who have previously been treated with at least one therapy
  • in combination with drugs bortezomib, melphalan, and prednisone (D-VMP) in newly diagnosed patients who are not eligible for autologous stem cell transplant (ASCT)
  • in combination with medicines bortezomib, thalidomide, and dexamethasone (D-VTd) in newly diagnosed patients who are eligible for autologous stem cell transplant (ASCT)
  • in combination with medicines bortezomib and dexamethasone in patients who have received at least one prior therapy
  • in combination with therapeutic drugs carfilzomib and dexamethasone in patients with relapsed or refractory multiple myeloma (RRMM) who have been treated with 1 to 3 prior lines of therapy
  • in combination with medicines pomalidomide (D-Pd) and dexamethasone in patients who have previously been treated with at least a couple of treatments including lenalidomide (D-Rd) and a proteasome inhibitor (PI)
  • as monotherapy, in patients who have been treated with at least three prior lines of treatment including a PI and an immunomodulatory agent, or who are double-refractory to a proteasome inhibitor (PI) and an immunomodulatory agent.

Dosage and Side Effects

The recommended dose of Daratumumab is 16 mg/kg actual body weight administered (in combination with other medications) as an intravenous (IV) infusion. Pre-medicate with corticosteroids, antipyretics, and antihistamines. Administer post-infusion medications.

The most common side effects of Daratumumab are infusion-related reactions, cough, upper respiratory infection, peripheral edema, neutropenia, constipation, thrombocytopenia, diarrhea, anemia, peripheral sensory neuropathy, dyspnea, fatigue, pyrexia, nausea, and asthenia.

Warning and Precautions

  • Daratumumab can cause serious IRRs including anaphylactic reactions. Pre-medicate patients with antipyretics, antihistamines, and corticosteroids. Monitor patients throughout the infusion. Interrupt infusion for reactions of any severity and institute medical management as required. Permanently stop therapy if an anaphylactic reaction or critical (Grade 4) reaction occurs and institute apt emergency care.
  • Daratumumab 100 mg/5 mL or 400 mg/20 mL may be responsible for increased neutropenia induced by background therapy. Monitor complete blood cell counts periodically throughout therapy as per the manufacturer’s PI for background therapies. Assess patients with neutropenia for signs and symptoms of infection. Consider withholding treatment until the full recovery of neutrophils.
  • Treatment with Daratumumab injection may increase thrombocytopenia induced by background therapy. Monitor complete blood cell counts (CBC) periodically throughout treatment according to the manufacturer’s PI for background therapies. Consider withholding this treatment until the full recovery of platelets. 
  • Based on the mechanism of action, the medicine Daratumumab can cause fetal harm if given to a pregnant woman. It may cause depletion of fetal immune cells and scaled-down bone density. Females of reproductive age must use suitable contraception during treatment and for 3 months after the last dose.
