Velpanat Tablet is a fixed-dose combination (FDC) drug of sofosbuvir, a hepatitis C virus (HCV) nucleotide analog NS5B polymerase inhibitor, and velpatasvir, an HCV NS5A inhibitor, and is approved for the treatment of adults and pediatric patients aged 3 years and older with chronic hepatitis C virus (HCV) genotype 1, 2, 3, 4, 5, or 6 infection without cirrhosis or with compensated cirrhosis, or with decompensated cirrhosis for use together with ribavirin.

Dosage and Side Effects

The recommended dosage of Velpanat in adults is one tablet (sofosbuvir 400 mg and velpatasvir 100 mg) taken by mouth once daily with or without food. When used with Velpanat, the recommended dosage of the drug ribavirin is based on the body weight (administered with food): 1,000 mg daily for patients not more than 75 kg and 1,200 mg for those weighing at least 75 kg, divided and given twice daily.

The recommended dosage of Velpanat in pediatric patients aged three years and older is based on body weight. Patients weighing less than 17 kg can take 150 mg/37.5 mg daily, patients weighing 17 kg to less than 30 kg can take 200 mg/50 mg daily, and those weighing at least 30 kg can take one 400 mg/100 mg tablet once daily with or without food.

The most common side effects of Sofosbuvir/Velpatasvir tablets are fatigue, nausea, anemia, headache, insomnia, vomiting, and diarrhea.

Warning and Precautions

  • Test all the respective patients for evidence of current or previous HBV infection by measuring HBsAg (hepatitis B surface antigen) and anti-HBc (hepatitis B core antibody) before initiating HCV therapy with sofosbuvir and velpatasvir.
  • Cardiac monitoring in an in-patient setting for the initial 48 hours of coadministration is needed, following which outpatient or self-monitoring of the heart rate should be done on a daily basis through at least the initial couple of weeks of treatment.
  • Counsel patients about the probability of symptomatic bradycardia. Those who develop signs or symptoms of bradycardia must seek medical assessment immediately. Symptoms may include dizziness, fainting, malaise, weakness, tiredness, chest pains, shortness of breath, or confusion.
  • Avoid using velpanat and ribavirin combination regimens in those for whom the drug ribavirin is contraindicated. Certain adverse reactions may occur with this combination regimen.
