Cabozantinib is a prescription medicine called a tyrosine kinase inhibitor (TKI) used for the treatment of:

  • patients with advanced renal cell carcinoma (RCC). 
  • patients with advanced renal cell carcinoma, as a first-line treatment in combination with nivolumab.
  • patients with hepatocellular carcinoma (HCC) who have previously been treated with medicine sorafenib.
  • adult and pediatric patients aged 12 years and older with locally advanced or metastatic differentiated thyroid cancer (DTC) that has progressed following prior VEGFR-targeted therapy and who are radioactive iodine-refractory or ineligible.

Dosage and Side Effects

Renal Cell Carcinoma (RCC): The recommended dosage of Cabozantinib tablets for Renal Cell Carcinoma (RCC) as a single agent is 60 mg and in combination with nivolumab is 40 mg once daily without food until disease progression or unacceptable toxicity. Nivolumab is used as 240 mg every 2 weeks (30-minute IV infusion) or 480 mg every 4 weeks (30-minute IV infusion).

Hepatocellular Carcinoma: The recommended dosage for patients with Hepatocellular Carcinoma as a single agent is 60 mg once daily until disease progression or unacceptable toxicity administered as recommended.

Differentiated Thyroid Cancer (DTC): The recommended dosage for patients with Differentiated Thyroid Cancer (DTC) as a single agent for adult and pediatric patients aged 12 years and older with BSA greater than or equal to 1.2 m2 is 60 mg once daily until disease progression or unacceptable toxicity administered as recommended.

The recommended dosage of Cabozantinib as a single agent in pediatric patients aged 12 years and older with BSA less than 1.2 m2 is 40 mg once daily until disease progression or unacceptable toxicity administered as recommended.

The most common side effects of Cabozantinib in patients with renal cell carcinoma (RCC), hepatocellular carcinoma (HCC), and differentiated thyroid cancer (DTC) are as follows:

  • as a single agent: nausea, weight decrease, constipation, vomiting, diarrhea, PPE, decreased appetite, fatigue, hypertension.

  • In combination with nivolumab: musculoskeletal pain, decreased appetite, hepatotoxicity, PPE, diarrhea, hypothyroidism, nausea, fatigue, stomatitis, rash, hypertension, dysgeusia, abdominal pain, cough, and upper respiratory tract infection.

Warning and Precautions

  • Severe and fatal hemorrhages may develop with Cabozantinib 20 mg/40 mg/60 mg. Discontinue Cabozantinib for Grade 3/4 hemorrhage. Do not administer this therapeutic drug to patients who have a recent history of hemorrhage, including hematemesis, hemoptysis, or melena.  

  • Fistulas and gastrointestinal (GI) perforations, including fatal cases, may occur in patients treated with Cabozantinib. Monitor patients for fistulas and perforations, including abscess and sepsis. Stop treatment in patients who experience a Grade 4 fistula or a gastrointestinal (GI) perforation.

  • Cabozantinib can cause hypertension, including hypertensive crisis. Do not initiate this treatment in patients with uncontrolled hypertension. Monitor blood pressure regularly during treatment.

  • Cabozantinib in combination with nivolumab can cause hepatotoxicity with higher frequencies of Grades 3/4 ALT and AST elevations compared to Cabozantinib alone. Monitor liver enzymes prior to initiating and periodically throughout treatment.

  • Proteinuria may occur in patients receiving Cabozantinib 60 mg/40 mg/20 mg therapy. Monitor urine protein regularly during treatment. Discontinue the use of this medicinal product in those who develop nephrotic syndrome.
