Hepcinat LP, a generic version of Harvoni, is a fixed-dose combination of Ledipasvir 90 mg, a hepatitis C virus NS5A inhibitor, and Sofosbuvir 400 mg, an HCV nucleotide analog NS5B polymerase inhibitor. This FDC medicine is used for the treatment of chronic hepatitis C virus (HCV) in adult and pediatric patients aged 3 years and older:

  • Genotype 1/4/5/6 infection either without cirrhosis or with compensated cirrhosis.
  • Genotype 1 infection with decompensated cirrhosis, along with ribavirin.
  • Genotype 1/4 infection who are the recipients of liver transplants either without cirrhosis or with compensated cirrhosis, along with ribavirin.

Dosage and Side Effects

The recommended dosage in adult patients with genotype 1/4/5/6 HCV is one tablet (ledipasvir 90 mg and sofosbuvir 400 mg) taken by mouth once daily with or without food. The daily dosage of ribavirin is weight-based (1000 mg for those <75 kg and 1200 mg for those ≥75 kg) administered by mouth in two divided doses with food. In those with decompensated cirrhosis, the beginning dose of ribavirin is 600 mg and may be titrated up to 1000 mg for individuals <75 kg and 1200 mg for individuals ≥75 kg in two divided doses with food.

The recommended dosage in pediatric patients aged 3 years and older with genotype 1/4/5/6 HCV using Hepcinat LP tablets is based on weight. Patients weighing  35 kg can take one 90 mg/400 mg tablet once daily, patients weighing 17 kg to less than 35 mg can take one 45 mg/200 mg tablet once daily, patients weighing less than 17 kg can take 33.75 mg/150 mg daily orally with or without food.

The most common side effects of Ledipasvir and Sofosbuvir tablets are headache fatigue, and asthenia.

Warning and Precautions

  • Test patients for the presence of any ongoing or previous HBV infection by measuring hepatitis B surface antigen (also known as HBsAg) and hepatitis B core antibody (also called anti-HBc) prior to starting hepatitis C virus treatment with Hepcinat LP tablets.
  • Symptomatic bradycardia, and serious cardiac arrest that may require pacemaker intervention, may establish when amiodarone is coadministered with Ledipasvir-Sofosbuvir. Coadministration of medicine amiodarone with Ledipasvir and Sofosbuvir is not recommended. 
  • The concomitant use of Hepcinat LP and P-gp inducers (such as St. John’s wort and rifampin) may significantly decrease the plasma concentrations of ledipasvir and sofosbuvir and may lead to a reduced therapeutic effect of Hepcinat LP tablets. Therefore, the use of this medicine with P-gp inducers (such as St. John’s wort or rifampin) is not recommended.
  • Patients taking Ledipasvir and Sofosbuvir who develop signs or symptoms of bradycardia need to seek prompt medical assessment. Symptoms may include shortness of breath, fainting, malaise, chest pains, dizziness or lightheadedness, weakness, tiredness, and memory problems.
