SoviHep V is a fixed-dose combination (FDC) of sofosbuvir, a hepatitis C virus (HCV) nucleotide analog NS5B polymerase inhibitor, and velpatasvir, an HCV NS5A inhibitor, and is approved for the treatment of adults and pediatric patients aged three years and older with chronic hepatitis C virus (HCV) genotype 1/2/3/4/5/6 infection without cirrhosis or with compensated cirrhosis, or with decompensated cirrhosis for use in conjunction with ribavirin.

Dosage and Side Effects

The recommended dose in adults is one tablet (sofosbuvir 400 mg and velpatasvir 100 mg) taken orally once daily with or without food/meal. When used with SoviHep V, the recommended dosage of the drug ribavirin depends on the body weight (administered with meal): 1,000 mg daily for those not more than 75 kg and 1,200 mg for patients weighing at least 75 kg, divided and administered twice daily.

The recommended dose in pediatric patients aged 3 years and older depends on body weight. Patients weighing less than 17 kg may take 150 mg/37.5 mg once daily, patients weighing 17 kg to less than 30 kg may take 200 mg/50 mg once daily, and those weighing minimum 30 kg may take one 400 mg/100 mg tablet once daily with or without a meal/food.

The most common side effects of Sofosbuvir 400 mg/Velpatasvir 100 mg are fatigue, nausea, anemia, headache, insomnia, vomiting, and diarrhea.

Warning and Precautions

  • All patients must be tested for evidence of ongoing or previous HBV infection by assessing HBsAg or hepatitis B surface antigen and anti-HBc or hepatitis B core antibody before initiating hepatitis C virus therapy with sofosbuvir-velpatasvir.
  • Counsel all the patients about the risk of symptomatic bradycardia. Those with signs/symptoms of bradycardia need to seek prompt medical assessment. Signs/symptoms may include fainting, dizziness, malaise, weakness, chest pains, tiredness, shortness of breath, and confusion.
  • Do not administer sofosbuvir-velpatasvir and ribavirin combination regimens in individuals for whom ribavirin drug is contraindicated. Certain side effects may develop with this combination treatment.
  • Cardiac monitoring in the in-patients setting for the first 48 hours of coadministration is needed, after which outpatient or self-monitoring of the heart rate must be done on a continuous basis through at least the initial two weeks of therapy.
