Thiotepa, an alkylating agent, is supplied generically under the brand Thioplan. This therapeutic drug is used:

  • To mitigate the risk of graft rejection when used in conjunction with high-dose busulfan and cyclophosphamide as a preparative regimen for allogeneic hematopoietic progenitor (stem) cell transplantation (HSCT) in pediatric patients with class 3 beta-thalassemia.
  • For the management of adenocarcinoma affecting the breast or ovary.
  • For managing intracavitary effusions arising from diffuse or localized neoplastic conditions affecting different serosal cavities.
  • For treatment of superficial papillary carcinoma of the urinary bladder.

Dosage & Side-effects

For class 3 beta-thalassemia, the recommended dose of Thiotepa is two intravenous administrations of 5 mg/kg each, approximately 12 hours apart on Day -6 before allogeneic HSCT, combined with high-dose busulfan and cyclophosphamide.

For adenocarcinoma of the breast or ovary, the recommended intravenous dose of Thiotepa ranges from 0.3 to 0.4 mg/kg.

For the treatment of malignant effusions, the recommended intracavitary dose of Thiotepa ranges from 0.6 mg/kg to 0.8 mg/kg.

For treating superficial papillary carcinoma of the urinary bladder, the recommended dose of Thiotepa is 60 mg administered into the bladder via a catheter in 30 to 60 mL of Sodium Chloride Injection.

The most common side effects of Thiotepa injection are hemorrhage, elevated alanine aminotransferase, anemia, diarrhea, neutropenia, thrombocytopenia, elevated aspartate aminotransferase, rash, elevated bilirubin, cytomegalovirus infection, mucositis, and hematuria.

  • Administering high doses of Thiotepa alongside other chemotherapy as prescribed for class 3 beta-thalassemia can lead to significant myelosuppression in all patients.   • In the event of clinically significant hypersensitivity reactions, including anaphylaxis, associated with Thiotepa usage, discontinue treatment, administer appropriate therapy, and closely monitor patients until resolution of signs and symptoms.
  • In the event of accidental exposure to Thiotepa 15 mg treatment, skin reactions may occur. Rinse the affected skin thoroughly with soap and water. If the solution comes into contact with mucous membranes, flush them immediately.
