Description

Lasinone (Nitisinone Capsules 2mg/5mg/10mg) is used to treat adult and pediatric patients (in any age range) with confirmed diagnosis of HT-1 (Hereditary tyrosinemia type-1 or Tyrosinemia type-1) in conjunction with dietary restriction of phenylalanine and tyrosine.

Dosage and Side Effects

The recommended starting dosage of Lasinone is 0.5 mg per kg administered orally twice daily at least 60 minutes prior to, or 120 minutes after taking a meal. In patients aged 5 years and older with undetectable serum and urine succinylacetone concentrations after at least 4 weeks on a stable dosage of Lasinone, the total daily dose may be given once daily (e.g., 1 mg/kg to 2 mg/kg once daily). 

Dietary restriction of tyrosine, as well as phenylalanine, needs to be maintained. The dosage of Lasinone can be titrated in each individual patient depending on biochemical and/or clinical response.

The most commonly reported side effects of Nitisinone capsules include: conjunctivitis, eye pain, dry skin, elevated tyrosine levels, thrombocytopenia, leukopenia, corneal opacity, photophobia, blepharitis, cataracts, keratitis, granulocytopenia, epistaxis, exfoliative dermatitis, maculopapular rash, pruritus, and alopecia.

Warning and Precautions

  • Nitisinone 10 mg/5 mg/2 mg capsule is an inhibitor of HPPD, also known as α-ketoisocaproate dioxygenase (KIC dioxygenase), an enzyme that exist in the tyrosine metabolic pathway. So, treatment with Lasinone may be responsible for causing an elevation in plasma tyrosine levels in those with HT-1.
  • Ocular manifestations including conjunctivitis, corneal opacities, corneal ulcers, photophobia, eye pain, and keratitis may appear in those treated with Nitisinone therapy. So, ophthalmologic evaluation including slit-lamp evaluation needs to be done prior to initiating Nitisinone therapy and regularly thereafter.
  • Patients treated with Lasinone 10 mg, 5 mg, and 2 mg and dietary restriction may develop transient leukopenia, thrombocytopenia, or both Patients must be monitored for platelet and white blood cell counts during Lasinone therapy.
  • Oral glycerol of 10 grams or more can be responsible for causing an upset stomach, headache, and diarrhea. Lasinone oral suspension generally contains 500 mg/mL of glycerol. Those taking more than 20 mL of Lasinone oral suspension (10 grams glycerol) as a single dose are at higher risk of these side effects. Consider switching those who are not able to tolerate the oral suspension to Lasinone capsules.
  • Patients taking treatment with Nitisinone capsules should be monitored for plasma and/or urine succinylacetone concentrations, alpha-fetoprotein levels, and liver function parameters. Evaluate dietary tyrosine as well as phenylalanine intake.
  • Use Lasinone during pregnancy only if the potential benefit justifies the possible risk to the fetus. Nursing women should discontinue either Lasinone treatment or breastfeeding on behalf of the recommendation of their health specialist.
