Sofosbuvir, a hepatitis C virus (HCV) nucleotide analog NS5B polymerase inhibitor, is supplied under the generic brand name Hepcinat. This therapeutic drug is widely used for the treatment of:

  • Adults with genotype 1, 2, 3 or 4 chronic HCV (hepatitis C virus) infection without cirrhosis or with compensated cirrhosis as a component of a combination antiviral treatment regimen.
  • Pediatric patients aged 3 years and older with genotype 2/3 chronic (hepatitis C virus) HCV infection without cirrhosis or with compensated cirrhosis in conjunction with ribavirin.

Dosage and Side Effects

The recommended dosage of Sofosbuvir is one 400 mg tablet, taken orally, once daily with or without food. It is taken in combination with ribavirin or in combination with pegylated interferon and ribavirin for treating hepatitis C virus (HCV).

The recommended dose of Sofosbuvir tablets in pediatric patients aged 3 years and older with genotype 2/3 hepatitis C virus (HCV) is based on weight. Patients weighing at least 35 kg should take one 400 mg tablet once daily, patients weighing 17 kg to less than 35 kg should take one 200 mg tablet once daily, and patients weighing less than 17 kg should take 150 mg daily with or without food.

The most common side effects of Sofosbuvir in combination with ribavirin are fatigue and headache, and in combination with medicines peginterferon alfa and ribavirin are fatigue, nausea, headache, insomnia, and anemia.

Warning and Precautions

  • Hepatitis B virus (HBV) reactivation may establish in HCV/HBV coinfected individuals who are undergoing or finished treatment with HCV direct acting antivirals, and who are not receiving HBV antiviral treatment.
  • Test patients for evidence of current or previous HBV infection by measuring HBsAg and anti-HBc prior to starting HCV treatment with Sofosbuvir 400 mg. In those with serologic presence of HBV infection, evaluate for clinical and laboratory signs of hepatitis flare or HBV reactivation during therapy and during post-treatment follow-up. Start useful patient management for HBV infection as clinically required.
  • Patients who are taking Hepcinat who need to begin amiodarone therapy because of no other alternative, viable therapy options should undergo cardiac monitoring. Because of amiodarone’s long half-life, those discontinuing amiodarone just before starting Hepcinat should also undergo cardiac monitoring.
  • Therapeutic drugs that are P-gp inducers in the intestine (such as St. John’s wort and rifampin) may significantly reduce sofosbuvir plasma concentrations and may reduce the therapeutic effect of Sofosbuvir. The use of rifampin and St. John’s wort with Sofosbuvir 400 mg tablets is not recommended.
