  • Vancomycin-resistant Enterococcus faecium infections, including concurrent bacteremia: The recommended dose for vancomycin-resistant Enterococcus faecium infections, including concurrent bacteremia, is 600 mg administered IV or orally every 12 hours. Treatment duration is extended for a period of 14 to 28 days.
  • Uncomplicated skin and skin structure infections: Adults should take a recommended dose of 400 mg orally every 12 hours. Treatment duration typically is 10 to 14 days.

The most common side effects of Linezolid are decreased platelets, hemoglobin, white blood cell counts, elevated pancreatic enzymes, headache, nausea, diarrhea, elevated liver function tests, and neuropathy.

Warning and Precautions

  • Linezolid has been associated with myelosuppression (including anemia, leukopenia, pancytopenia, and thrombocytopenia). It is important to monitor complete blood counts weekly, especially in patients who have preexisting myelosuppression, are taking concomitant medications that may depress hemoglobin levels, platelet counts, or both, or who have a chronic infection and have been receiving Linezolid for more than 2 weeks.
  • Linezolid is a reversible, nonselective inhibitor of monoamine oxidase and can interact with serotonergic agents, potentially leading to serotonin syndrome. Symptoms include mental status changes, hyperthermia, autonomic instability, and neuromuscular abnormalities. Concomitant use with SSRIs, SNRIs, and other serotonergic agents should be avoided unless absolutely necessary.
  • Prolonged use of Linezolid (more than 28 days) has been associated with peripheral and optic neuropathy. Regular monitoring of vision is recommended for patients receiving Linezolid for extended periods.
  • Treatment with this medicine can cause hypoglycemia, particularly in diabetic patients on insulin or oral hypoglycemic agents. Blood glucose levels should be closely monitored in such patients during treatment.
  • There have been reports of lactic acidosis associated with the use of this medication. Symptoms include recurrent nausea and vomiting, unexplained acidosis, or a low bicarbonate level. Patients should be monitored for signs and symptoms of lactic acidosis during treatment.
  • Because Linezolid is a reversible MAO inhibitor, it can cause hypertension if taken with large quantities of tyramine-rich foods or sympathomimetic agents. Patients should be advised to avoid high-tyramine foods and drugs that can raise blood pressure.
  • As with other antibiotics, prolonged use of Linezolid may result in overgrowth of nonsusceptible organisms, including fungi. Regular assessment of the patient is essential to identify potential superinfections, and appropriate measures should be taken if a superinfection occurs.
