The recommended dosing regimen for treating Plaque psoriasis, Psoriatic arthritis, and Behcet disease is initiated as a 5-day titration period, gradually increasing the dose of apremilast over the initial five days until the recommended dose is reached.
Day 1 – 10 mg administered in the morning once daily
Day 2 – 10 mg administered in the morning and 10 mg administered in the evening
Day 3 – 10 mg administered in the morning and 20 mg administered in the evening
Day 4 – 20 mg administered in the morning and 20 mg administered in the evening
Day 5 – 20 mg administered in the morning and 30 mg administered in the evening
Day 6 – 30 mg administered twice daily moving forward
During the treatment initiation, patients need to be started on an Apremilast 10 mg dosage, followed by gradual dosage titration by an increase of 10 mg daily for 6 days, reaching the optimal daily dosage of 30 mg twice daily by day six. Dosage can be administered without regard to meals. Never split, crush, or chew the tablets.Â
The most frequently reported side effects of apremilast are diarrhea, nausea, depression, and suicidal ideations and behaviors.
Hypersensitivity reactions, including cases of angioedema and anaphylaxis, may occur. Avoid the use of apremilast 30 mg in those with known hypersensitivity to this medicine or to any of the excipients in the formulation. If signs or symptoms of serious hypersensitivity reactions develop during treatment, discontinue therapy and institute suitable therapy.Â
Cases of severe diarrhea, vomiting, and nausea may occur with the use of apremilast. Monitor patients who are a bit more susceptible to complications of vomiting or diarrhea. Patients who reduced dosage or discontinued this treatment generally improved promptly.
Treatment with this medicine is related to an increased incidence of depression. Prior to using this medicine in patients with a known history of depression or suicidal thoughts or behavior, prescribers should precisely weigh the risks as well as benefits of therapy with apremilast pills.
Patients treated with apremilast tablets must have their weight monitored on a regular basis. In case an unexplained or clinically significant weight loss occurs, weight loss must be evaluated, and discontinuation of treatment should be considered.
Co-administration of strong cytochrome P450 enzyme inducer, rifampin, may cause a reduction of systemic exposure to apremilast, which may cause a loss of efficacy of Arosis-P 30. Therefore, cytochrome P450 enzyme inducers (e.g., phenobarbital, rifampin, carbamazepine, phenytoin) with Arosis-P 30 is not recommended.
