Botulinum Toxin is an acetylcholine release inhibitor and a neuromuscular blocking agent used for:

  • Treatment of urinary incontinence because of detrusor overactivity associated with a neurologic condition (such as spinal cord injury, or multiple sclerosis) in those who have an inadequate response to or are intolerant of an anticholinergic medicine.

  • Prophylaxis of headaches in adult patients with chronic migraine (≥15 days per month with headache lasting four hours a day or longer).

  • Treatment of upper limb spasticity in adult patients.

  • Treatment of cervical dystonia in adult patients, to reduce the severity of abnormal head position and neck pain.

  • Treatment of severe axillary hyperhidrosis that is inadequately managed by topical agents in adult patients.

  • Treatment of blepharospasm associated with dystonia in patients aged ≥12 years.

  • Treatment of strabismus in patients aged ≥12 years.

Dosage and Side Effects

Follow the indication-specific dosage and administration. Do not exceed a total Botulinum dose of 360 Units administered in a three-month interval.

  • Detrusor Overactivity associated with a Neurologic Condition: The recommended total dose is 200 Units, as 1 mL (~6.7 Units) injections across 30 sites into the detrusor.

  • Chronic Migraine: The recommended total dose is 155 Units, as 0.1 mL (5 Units) injections per each site divided across 7 head/neck muscles.

  • Upper Limb Spasticity: Select dose on behalf of muscles affected, severity of muscle activity, prior response to therapy, and side effect history.

  • Cervical Dystonia: Base dosing on the patient’s head and neck position, localization of pain, muscle hypertrophy, patient response, and side effect history; use lower initial dose in botulinum toxin naĂ¯ve patients.

  • Axillary Hyperhidrosis: The recommended dosage is 50 Units per axilla.

  • Blepharospasm: The recommended dosage is 1.25 to 2.5 Units into each of 3 sites per affected eye.

  • Strabismus: The recommended dosage is 1.25 to 2.5 Units initially in any one muscle.

The most common side effects of Botulinum Toxin are urinary tract infection, urinary retention, neck pain, headache, pain in extremity, dysphagia, upper respiratory infection, headache, increased cough, neck pain, flu syndrome, back pain, rhinitis, injection site pain and hemorrhage, non-axillary sweating, pharyngitis, and flu syndrome.

Warning and Precautions

  • Serious or immediate hypersensitivity reactions may occur. These reactions include anaphylaxis, soft tissue edema, serum sickness, urticaria, and dyspnea. In case such a reaction occurs, further injection of Botulinum Toxin should be stopped and appropriate medical therapy promptly started.

  • Therapy with Botox injection and other botulinum toxin products can cause swallowing or breathing complications. Those with preexisting swallowing or breathing complications may be more susceptible to these difficulties Those treated with botulinum toxin may need immediate medical attention if they develop problems with swallowing, speech, or respiratory disorders.

  • Cervical dystonia treatment with botulinum toxins may weaken neck muscles that serve as accessory muscles of ventilation. This may cause a critical loss of breathing capacity in those with respiratory disorders who may have become dependent upon these accessory muscles.

  • Those with peripheral motor neuropathic diseases/complications, amyotrophic lateral sclerosis or neuromuscular junction disorders should be monitored closely when given botulinum toxin. Those with neuromuscular disorders may be at higher probability of clinically significant effects including severe dysphagia and respiratory compromise from therapeutic doses of Botox.
