Description

Cefazolin injection is a cephalosporin antibacterial used for the treatment of perioperative prophylaxis in adults and certain infections (respiratory tract infections, urinary tract infections, biliary tract infections, skin and skin structure infections, bone and joint infections, genital infections, septicemia, and endocarditis) caused by susceptible isolates of the designated microorganisms in adult and pediatric patients aged 1 month and older for whom appropriate dosing with this formulation can be achieved.

Dosage and Side Effects

Cefazolin is for intravenous (IV) infusion only. Do not use it for intravenous (IV) bolus administration or intramuscular administration. The recommended dosage in adult patients with CLcr that is greater than or equal to 55 mL/min is as follows:

  • Moderate to severe infections: 500 mg to 1 gram administered every 6-8 hours 

  • Mild infections caused by susceptible gram-positive cocci: 250-500 mg administered every 8 hours

  • Acute, uncomplicated urinary tract infections: 1 gram administered every 12 hours

  • Pneumococcal pneumonia: 500 mg administered every 12 hours

  • Severe, life-threatening infections (e.g., endocarditis, septicemia): 1 gram to 1.5 grams every 6 hours

  • Perioperative prophylaxis: 1 gram (administered 30 minutes to 1 hour prior to the start of surgery), 500 mg to 1 gram (administered during surgery for lengthy procedures such as 2 hours or more), and 500 mg to 1 gram (administered every 6-8 hours for 24 hours postoperatively).

The recommended dosage in pediatric patients with CLcr that is greater than or equal to 70 mL/min is as follows: 

  • Mild to moderately severe infections: 25 mg/kg to 50 mg/kg divided into 3/4 equal doses 

  • Severe infections: May increase to 100 mg/kg divided into 3/4 equal doses.

The most common side effects of Cefazolin in adult and pediatric patients are gastrointestinal (nausea, vomiting, diarrhea), and allergic reactions (anaphylaxis, skin rash).

Warning and Precautions

  • Serious and sometimes fatal hypersensitivity (anaphylactic) reactions may occur in patients receiving beta-lactam antibacterial medicines. If an allergic reaction to Cefazolin injection occurs, discontinue the medicine.  Exercise caution if this medicine is to be given to penicillin-sensitive patients.

  • Seizures may develop with the use of Cefazolin injection, specifically in individuals with renal impairment when the dosage is not decreased appropriately. Discontinue treatment in case seizures occur or make relevant dosage adjustments in those with renal impairment.

  • Prescribing Cefazolin in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to offer benefit to the patient and increases the probability of the development of drug-resistant bacteria. Prolonged use of this therapeutic drug may cause overgrowth of nonsusceptible organisms. Precise clinical observation of the patient is essential.

  • Cephalosporins may be related to a fall in prothrombin activity. Prothrombin time needs to be monitored in individuals at risk and exogenous vitamin K is administered as indicated.
