Description

Eplerenone tablet is an antagonist of aldosterone that is used in: 

  • Enhancing the prognosis of stable individuals suffering from an acute myocardial infarction who have congestive heart failure (CHF) and LV systolic dysfunction (LVEF ≤40%). 
  • Hypertension, either on its own or in conjunction with other substances.

Dosage and Side Effects

CHF Post-MI: As soon as the patient is able to tolerate it, start the medication at a dose of 25 mg once a day and work your way up to the recommended 50 mg once daily, ideally within 4 weeks. You can take the eplerenone pills with or without food.

Hypertension: Eplerenone should be started at a starting dose of 50 mg once daily. It takes four weeks for eplerenone to fully take action as a medication. Eplerenone dosages should be increased to 50 mg twice daily for those whose blood pressure does not respond well to 50 mg once daily. Since higher doses of eplerenone come with a higher risk of hyperkalemia and don’t affect blood pressure any more than 100 mg, they are not advised.

Warning and Precautions

  • Serum potassium should be measured before starting eplerenone 25 mg tablet medication, during the first week, and one month after treatment initiation or dose modification.

  • Tablet eplerenone should not be used to treat hypertensive patients with serum creatinine levels >2.0 mg/dL (males) or >1.8 mg/dL (females) or creatinine clearance ≤50 mL/min.

  • Patients with diabetes who are on Eplerenone therapy and who have developed CHF after a MI should be handled carefully, particularly if they have proteinuria.

  • Avoid using potassium supplements or potassium-containing salt substitutes while using eplerenone without first speaking with the prescribing medical professionals.

  • Up until the impact of eplerenone is determined, keep an eye out for or assess patients for the development of hyperkalemia.

  • During the course of treatment, women on eplerenone 25 should refrain from getting pregnant and from nursing.
