Macitentan tablet is an endothelin receptor antagonist (ERA) used for treating pulmonary arterial hypertension (PAH is a high blood pressure in the arteries of the lungs). This therapeutic drug can improve the ability to exercise as measured by the 6-minute walk distance, can improve some of the symptoms, can help slow down the progression of the disease, and can reduce the chance/probability of being hospitalized for pulmonary arterial hypertension (PAH).
The recommended dosage of Macitentan tablets is 10 mg administered orally once daily. Doses higher than 10 mg once daily have not been studied in patients with pulmonary arterial hypertension (PAH) and are not recommended.
The most common side effects of Macitentan 10 mg tablets are anemia, nasopharyngitis/pharyngitis, influenza, urinary tract infection, headache, and bronchitis.
Macitentan 10 mg tablet may cause fetal harm if administered during pregnancy and is contraindicated for use in women who are pregnant. In women of reproductive potential/age, exclude pregnancy before initiating therapy, ensure the use of acceptable contraceptive methods, and obtain monthly pregnancy tests.
Other ERAs may cause elevations of aminotransferases, hepatotoxicity, and liver failure. Obtain liver enzyme tests before starting treatment with Macitentan 10 mg tablets and repeat during treatment as clinically required. Patients should report symptoms suggesting hepatic injury (nausea, right upper quadrant pain, vomiting, fatigue, dark urine, anorexia, jaundice, fever, or itching).
Reductions in hemoglobin concentration and hematocrit may occur following administration of other ERA. Initiation of Macitentan is not recommended in those with severe anemia. Measure hemoglobin before initiating treatment and repeat during treatment as clinically required.
Should signs of pulmonary edema occur, consider the possibility of associated Pulmonary Veno-occlusive Disease (PVOD). If confirmed, discontinue treatment with Macitentan. Other ERAs may cause adverse effects on spermatogenesis. Counsel men about potential effects on fertility.
