Crizotinib is approved to treat certain types of lung cancer prevalent in humans. It is a kinase inhibitor used for the treatment of 

  • patients with metastatic non-small cell lung cancer (NSCLC) whose tumors are ALK/ROS1-positive. 
  • pediatric patients aged 12 months and older and young adults with relapsed or refractory, systemic anaplastic large cell lymphoma (ALCL) that is ALK-positive. 
  • adult and pediatric patients aged 12 months and older with unresectable, recurrent, or refractory inflammatory myofibroblastic tumor (IMT) that is ALK-positive.

Dosage and Side Effects

Metastatic NSCLC: The recommended dosage for patients with metastatic non-small cell lung cancer (NSCLC) is 250 mg. 

Systemic ALCL: The recommended dosage for patients with systemic anaplastic large cell lymphoma (ALCL) is 280 mg/m2 based on body surface area. 

Unresectable IMT: The recommended dosage for adults is 250 mg and 280 mg/m2 for pediatric patients based on body surface area. 

Take Crizotinib capsules orally twice daily with or without food. Swallow them whole without crushing, chewing, breaking, or opening. If a dose is missed, make up that dose unless the very next dose is due within 6 hours. If vomiting occurs after taking a dose, take the next scheduled dose at the regular time.

The most common side effects of Crizotinib in adult patients with NSCLC are nausea, diarrhea, vomiting, vision disorders, constipation, decreased appetite, elevated transaminases, fatigue, neuropathy, upper respiratory infection, edema, and dizziness. 

The most common side effects of Crizotinib in patients with ALCL are vomiting, diarrhea, nausea, vision disorder, musculoskeletal pain, headache, decreased appetite, stomatitis, fatigue, pyrexia, abdominal pain, cough, and pruritus.

The most common side effects of Crizotinib capsules in adult patients with IMT are nausea, edema, and vision disorders.

The most common side effects of Crizotinib in pediatric patients with IMT are nausea, vomiting, diarrhea, abdominal pain, cough, rash, vision disorder, pyrexia, musculoskeletal pain, fatigue, edema, constipation, upper respiratory tract infection, and headache.

Warning and Precautions

  • Drug-induced hepatotoxicity with fatal outcomes may occur. Monitor liver function tests, including AST, ALT, and total bilirubin, every 14 days during the initial 60 days of therapy, then once monthly, and as clinically required, with more frequent repeat testing for increased liver transaminases, alkaline phosphatase, or total bilirubin in individuals who develop increased transaminases. 
  • Severe, critical, or fatal interstitial lung disease (ILD) or pneumonitis can occur in those treated with Crizotinib 200 mg/250 mg. Monitor individuals for pulmonary signs and symptoms of ILD or pneumonitis. Exclude other probable causes of ILD or pneumonitis, and permanently stop this treatment in those diagnosed with drug-related ILD or pneumonitis.
  • QTc prolongation may be established in those treated with Crizotinib. Avoid the use of this drug in those with congenital long QT syndrome. Monitor electrolytes and ECGs in individuals with bradyarrhythmias, congestive heart failure, electrolyte abnormalities, or who are taking drugs that are known to prolong the QT interval.
  • Crizotinib capsules can cause serious gastrointestinal toxicities in both pediatric and young adult patients with ALCL or pediatric patients with IMT. Provide standard antidiarrheal and antiemetic agents for gastrointestinal toxicities in both pediatric and young adult patients with ALCL or pediatric patients with IMT. Antiemetics are recommended before and throughout treatment to prevent vomiting and nausea.
  • Based on its mechanism of action, this medicinal product can cause fetal harm if administered to a pregnant woman. Females of reproductive age or potential should use effective contraception during treatment and for at least 45 days following the final dose.
