Description

Eribulin Mesylate, supplied generically under the brand Ebunat, is a microtubule inhibitor used for the treatment of patients with:

  • Metastatic breast cancer who have previously been treated with at least two chemotherapeutic regimens for their metastatic disease. Before starting this therapy, patients should have already received a type of chemotherapy called an anthracycline and another type called a taxane.
  • Unresectable or metastatic liposarcoma who have previously been treated with an anthracycline-containing regimen. 

Dosage and Side Effects: The recommended dosage of Eribulin Mesylate is 1.4 mg per square meter of body surface administered intravenously (IV) over 2-5 minutes on Days 1 and 8 of a 21-day cycle. Reduce the dose of Eribulin in patients with hepatic impairment or with moderate or severe renal impairment. It is not advisable to mix it with other medicines or administer it with dextrose-containing solutions. 

The most common side effects of Eribulin Mesylate in metastatic breast cancer are anemia, neutropenia, nausea, fatigue, peripheral neuropathy, alopecia, and constipation.

The most common side effects of Eribulin Mesylate in liposarcoma and leiomyosarcoma are constipation, fatigue, nausea, alopecia, abdominal pain, peripheral neuropathy, and pyrexia.

Warning and Precautions:

  • Patients with alanine aminotransferase (ALT) or aspartate aminotransferase (AST) >3 x upper limit of normal (ULN) experienced an increased incidence of Grade 4 neutropenia and febrile neutropenia. Monitor complete blood counts (CBC) before each dose; increase the frequency of monitoring in those who develop Grade 3/4 cytopenias.
  • Peripheral neuropathy may develop in patients with metastatic breast cancer (MBC) receiving treatment with Eribulin. Monitor patients precisely for signs of peripheral motor and sensory neuropathy. Withhold Eribulin in those who experience Grade 3/4 peripheral neuropathy, until resolution to Grade 2 or less.
  • Findings and mechanism of action suggest that Eribulin 1 mg per 2 mL (0.5 mg per mL) can cause fetal harm if administered to a pregnant woman. Women who could become pregnant should use birth control during treatment and for 2 weeks after the last dose.
  • QT prolongation may occur. ECG monitoring is required if therapy is initiated in individuals with congestive heart failure, bradyarrhythmias, or drugs known to prolong the QT interval. Correct hypokalemia or hypomagnesemia before initiating Eribulin and monitor these electrolytes periodically during therapy. Avoid the use of Eribulin in those with congenital long QT syndrome.
