Velasof Tablet is a fixed-dose combination (FDC) of sofosbuvir, a hepatitis C virus (HCV) nucleotide analog NS5B polymerase inhibitor, and velpatasvir, an HCV NS5A inhibitor, and is used for the treatment of adults and pediatric patients aged 36 months and older with chronic hepatitis C virus (HCV) genotype 1/2/3/4/5/6 infection in absence of cirrhosis with compensated cirrhosis, or with decompensated cirrhosis for use along with ribavirin.

Dosage and Side Effects

The recommended dose of Velasof in adults is one 400 mg/100 mg tablet, taken orally, once daily with or without food. The recommended dose in pediatric patients aged 36 months and above is dependent on body weight. Patients with body weight ≥ 30 kg can take one 400 mg/100 mg tablet once daily for 12 weeks, and patients with body weight 17 kg to < 30 kg can take 200 mg/50 mg per day with or without food for 12 weeks.

The most common side effects of Sofosbuvir and Velpatasvir are fatigue, nausea, anemia, headache, insomnia, vomiting, and diarrhea.

Warning and Precautions

  • Critical cases of serious bradycardia and heart block may develop when sofosbuvir-containing regimens are used along with amiodarone. Bradycardia may generally develop within hours to days, but cases with a longer time to onset may occur mostly up to 2 weeks after initiating HCV therapy.
  • Cases of HBV reactivation, a few of them fatal, may occur throughout or after treatment with direct-acting antiviral medicines. HBV screening needs to be done in all patients before starting treatment. HCV/HBV co-infected individuals are at probability of HBV reactivation, and should, therefore, be evaluated and managed as per current clinical guidelines.
  • Medicines that are moderate P-gp or moderate CYP inducers (such as modafinil, oxcarbazepine, efavirenz, and/or rifapentine) may scale down sofosbuvir 400 mg or velpatasvir 100 mg plasma concentrations leading to decreased therapeutic effect of Velasof. Co-administration of such therapeutic drugs with Velasof is not recommended.
  • Diabetic patients may experience elevated glucose control, potentially resulting in symptomatic hypoglycemia, after starting HCV direct-acting antiviral therapy. Glucose levels of diabetic patients starting direct-acting antiviral treatment should be precisely monitored, mainly within the initial 3 months, and their diabetic therapy modified when required.
